對(duì)強(qiáng)心苷類(lèi)臨床應(yīng)用相關(guān)問(wèn)題的思考

【摘要】:強(qiáng)心苷類(lèi)藥物的主要藥理機(jī)制是抑制心肌細(xì)胞膜Na~+-K~+-ATP酶,干擾膜內(nèi)外相應(yīng)離子濃度,影響細(xì)胞生理功能,致Na~+-Ca~(2+)代償性交換增多,使心肌收縮性增強(qiáng)并表現(xiàn)相應(yīng)臨床效應(yīng)。但膜電位的減低使心肌細(xì)胞興奮性增高及傳導(dǎo)性下降,為心肌電紊亂提供了可能,也為其臨床應(yīng)用帶來(lái)諸多爭(zhēng)議。特別是當(dāng)電解質(zhì)紊亂和心律失常時(shí),對(duì)該藥的選擇和使用存在較多疑點(diǎn),而對(duì)該類(lèi)藥物藥理機(jī)制和心肌電生理的正確認(rèn)識(shí)和理解是臨床安全合理用藥的保證。
[Abstract]:The main pharmacological mechanism of cardioside drugs is to inhibit Na~ K- ATP enzyme of myocardial cell membrane, interfere with the corresponding ion concentration inside and outside the membrane, affect the physiological function of cells, and increase the compensatory exchange of Na~ Ca~ (2). The myocardial contractility was enhanced and the corresponding clinical effect was demonstrated. However, the decrease of membrane potential leads to the increase of excitability and the decrease of conduction of myocardial cells, which provides the possibility of myocardial electrical disturbance and brings many controversies to its clinical application. Especially when the electrolyte is in disorder and arrhythmia, there are many doubts about the choice and use of the drug, and the correct understanding and understanding of the pharmacological mechanism and myocardial electrophysiology is the guarantee of safe and reasonable drug use in clinic.
