我國(guó)現(xiàn)行藥品醫(yī)保談判機(jī)制運(yùn)行情況調(diào)查研究

【摘要】:目的:為我國(guó)藥品醫(yī)保談判機(jī)制的進(jìn)一步完善提供參考。方法:對(duì)參與江蘇省、江西省、青島市、成都市四地藥品醫(yī)保談判的部分醫(yī)保經(jīng)辦機(jī)構(gòu)和醫(yī)藥企業(yè)工作人員進(jìn)行問(wèn)卷調(diào)查,了解其業(yè)務(wù)熟悉情況和對(duì)談判機(jī)制的認(rèn)可程度,并采用描述性分析、獨(dú)立樣本t檢驗(yàn)分析等方法對(duì)數(shù)據(jù)進(jìn)行統(tǒng)計(jì)和分析。結(jié)果:共發(fā)放問(wèn)卷70份,回收有效問(wèn)卷68份,有效回收率為97.14%;包括醫(yī)保經(jīng)辦機(jī)構(gòu)受訪(fǎng)者21人,醫(yī)藥企業(yè)受訪(fǎng)者47人。對(duì)于業(yè)務(wù)熟悉情況,受訪(fǎng)者各方面評(píng)分平均值均3(4分制),以"藥品談判項(xiàng)目執(zhí)行過(guò)程中的管理方式"評(píng)分最低;兩類(lèi)受訪(fǎng)者在"藥品談判過(guò)程應(yīng)遵循的基本原則""實(shí)質(zhì)性談判過(guò)程中的談判目標(biāo)""業(yè)務(wù)熟悉情況總評(píng)分"方面的評(píng)分比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05),醫(yī)藥企業(yè)受訪(fǎng)者均高于醫(yī)保經(jīng)辦機(jī)構(gòu)受訪(fǎng)者。在對(duì)談判機(jī)制的認(rèn)可程度上,對(duì)談判機(jī)制整體效果的認(rèn)可程度除"有利于維持藥企的價(jià)格體系"外其余各方面評(píng)分平均值均≥4(5分制),對(duì)談判準(zhǔn)備工作、實(shí)質(zhì)性談判工作、談判協(xié)議執(zhí)行情況、談判項(xiàng)目監(jiān)管評(píng)估情況的認(rèn)可程度除"患者取藥用藥方便快捷"外其他各環(huán)節(jié)各方面評(píng)分平均值均4。兩類(lèi)受訪(fǎng)者對(duì)談判機(jī)制整體效果、談判準(zhǔn)備工作、談判項(xiàng)目監(jiān)管評(píng)估情況的認(rèn)可程度各方面評(píng)分比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);而對(duì)實(shí)質(zhì)性談判工作的認(rèn)可程度在"談判過(guò)程中雙方在平等、合作、融洽的氛圍下進(jìn)行談判"方面的評(píng)分比較差異有統(tǒng)計(jì)學(xué)意義(P0.05),醫(yī)保經(jīng)辦機(jī)構(gòu)受訪(fǎng)者高于醫(yī)藥企業(yè)受訪(fǎng)者;同時(shí),對(duì)談判協(xié)議執(zhí)行情況的認(rèn)可程度在"在協(xié)議生效后,醫(yī)保經(jīng)辦機(jī)構(gòu)嚴(yán)格按照協(xié)議執(zhí)行""基金向藥企方支付及時(shí)、方式合理"兩方面的評(píng)分比較差異均有統(tǒng)計(jì)學(xué)意義(P≤0.05),醫(yī)保經(jīng)辦機(jī)構(gòu)受訪(fǎng)者均高于醫(yī)藥企業(yè)受訪(fǎng)者。結(jié)論:現(xiàn)行藥品醫(yī)保談判機(jī)制整體效果較好,運(yùn)行較為順暢,但仍存在準(zhǔn)備階段收集的依據(jù)材料不夠充分、患者取藥用藥流程繁復(fù)、談判雙方地位不平等、基金向藥企方支付不及時(shí)、評(píng)估工作不完善等不足。
[Abstract]:Objective: to provide a reference for the further improvement of the negotiation mechanism of drug and medical insurance in China. Methods: a questionnaire survey was conducted among some medical insurance agencies and staff of pharmaceutical enterprises in Jiangsu, Jiangxi, Qingdao and Chengdu to find out their business familiarity and the degree of approval of the negotiation mechanism. Descriptive analysis, independent sample t-test analysis and other methods are used to analyze the data. Results: a total of 70 questionnaires were sent out and 68 valid questionnaires were collected, with an effective recovery rate of 97.14, including 21 respondents from medical insurance agencies and 47 respondents from pharmaceutical enterprises. For business familiarity, the average score of all aspects of the interviewees was 3 (4 points system), with the lowest score of "the management mode in the execution of drug negotiation project"; There were significant differences between the two groups of respondents in terms of "the basic principles to be followed in the Drug negotiation process", "the negotiation objectives in the substantive negotiation process" and "the General score of Business familiarity" (P0.05), while the respondents of pharmaceutical enterprises had significant differences (P0.05). They were higher than the respondents of medical insurance agencies.
