【摘要】：藥物消除半衰期是最常使用和最重要的藥物代謝動(dòng)力學(xué)參數(shù),一般采用單次靜脈注射后不同時(shí)間采血的方法測(cè)定。該法由于藥物分布的參與,存在藥物分布完成的時(shí)間難以確定,過(guò)多的失血影響消除,單次給藥的血藥濃度可能低于臨床實(shí)際,消除相血藥濃度低而誤差大,使測(cè)定的半衰期結(jié)果與實(shí)際可能會(huì)有較大距離。我們推薦一種基于單純消除的藥物半衰期測(cè)定方法,采用長(zhǎng)時(shí)間(至少5個(gè)半衰期)持續(xù)靜脈滴注,使給藥速度等于體內(nèi)藥物的消除速度,其分布到達(dá)平衡后,再停止給藥。此時(shí)血藥濃度的變化僅反映藥物的消除,于不同時(shí)間靜脈采血,再計(jì)算藥物的消除半衰期,這樣的結(jié)果更接近體內(nèi)藥物消除的實(shí)際狀況。
[Abstract]:Drug elimination half-life is the most commonly used and most important pharmacokinetic parameter. It is usually determined by blood collection at different times after a single intravenous injection. The time of completion of drug distribution is difficult to determine, too much blood loss can be eliminated, the blood drug concentration of single dose may be lower than that of clinical practice, and the elimination phase of blood drug concentration is low and the error is large. We recommend a drug half-life determination method based on simple elimination for a long (at least 5 half-life) duration of intravenous drip. So that the drug delivery rate is equal to the elimination speed of the drug in the body, the distribution of the drug is balanced and then the drug is stopped. At this time, the change of the blood drug concentration only reflects the elimination of the drug, the venous blood is collected at different times, and the elimination half life of the drug is calculated. The results are closer to the actual state of drug elimination in the body.
