醫(yī)院信息系統(tǒng)在抗菌藥物管理中的應(yīng)用研究

【摘要】:目的:本課題旨在借鑒國內(nèi)外臨床抗菌藥物管理方面的經(jīng)驗(yàn),針對國內(nèi)臨床抗菌藥物管理方面存在的問題,研究臨床抗菌藥物管理計(jì)算機(jī)網(wǎng)絡(luò)系統(tǒng)應(yīng)用于臨床的效果。開發(fā)醫(yī)院臨床抗菌藥物用藥情況查詢和分析軟件,利用計(jì)算機(jī)網(wǎng)絡(luò),快速檢索和統(tǒng)計(jì)全院抗菌藥品使用情況。通過分析臨床抗菌藥物的使用頻率以及抗菌藥物處方占比、聯(lián)合用抗菌藥頻率、抗菌藥物使用強(qiáng)度等指數(shù)評估醫(yī)院抗菌藥物管理水平。以期達(dá)到切實(shí)提高醫(yī)院抗菌藥物管理、最大限度地實(shí)現(xiàn)“以病人為中心”的醫(yī)療服務(wù)目的。所進(jìn)行的具體應(yīng)用有望使抗菌藥物管理水平能夠顯著提升,提高醫(yī)院抗菌藥物管理水平。 方法:通過調(diào)查、問詢,隨訪臨床專家,醫(yī)師及藥師等研究抗菌藥物使用現(xiàn)狀及原因;選取2013年7月至2014年6月期間,在某二級綜合醫(yī)院應(yīng)用抗菌藥物管理系統(tǒng)后為實(shí)施組,同時(shí)在信息庫中抽調(diào)出2012年7月至2013年6月期間在該院未應(yīng)用抗菌藥物管理系統(tǒng)前為對照組,兩組進(jìn)行比較;統(tǒng)計(jì)分析采用SPSS17.0和Excel2007統(tǒng)計(jì)軟件進(jìn)行數(shù)據(jù)分析。其中計(jì)量資料以均數(shù)和標(biāo)準(zhǔn)差(X2±S)表示,兩組之間的比較采用t檢驗(yàn);計(jì)數(shù)資料應(yīng)用χ檢驗(yàn)。差異的顯著性以P0.05為有統(tǒng)計(jì)學(xué)意義。 結(jié)果:信息化管理是醫(yī)院今后發(fā)展的必然趨勢,抗菌藥物管理系統(tǒng)(AMIS)的運(yùn)用,,大大優(yōu)化了醫(yī)院業(yè)務(wù)流程,提升了醫(yī)院管理水平,建立了以患者為中心的服務(wù)模式,實(shí)現(xiàn)了良好的社會效益、經(jīng)濟(jì)效益、管理效益,達(dá)到了醫(yī)院信息化建設(shè)的根本目標(biāo)。本研究顯示,應(yīng)用抗菌藥物管理系統(tǒng)后,實(shí)施組的門診抗菌藥物處方數(shù)較對照組降低了6.43%,實(shí)施組的抗菌藥物使用人次占出院人次比例較對照組降低了4.11%,實(shí)施組的I類切口手術(shù)抗菌藥物使用率較對照組降低了4.8%,實(shí)施組的住院患者抗菌藥物使用強(qiáng)度(DDD值)較對照組降低了28.88,經(jīng)檢驗(yàn)P值均0.05,具有統(tǒng)計(jì)學(xué)意義,提示兩組間有顯著差別。 結(jié)論:本文就抗菌藥物管理系統(tǒng)平臺的應(yīng)用背景和抗菌藥物管理系統(tǒng)的價(jià)值和方式進(jìn)行了總結(jié)概述,并闡述了通過實(shí)踐應(yīng)用系統(tǒng)平臺監(jiān)測抗菌藥物管理的主要效果。通過比較分析,驗(yàn)證了該項(xiàng)研究對抗菌藥物管理水平的推進(jìn)和顯著提升。臨床抗菌藥物管理計(jì)算機(jī)網(wǎng)絡(luò)系統(tǒng)為臨床抗菌藥物使用提供了有效的指導(dǎo)和監(jiān)測手段?咕幬锕芾硐到y(tǒng)已成為我院管理、臨床醫(yī)藥工作必不可少的工具,可為臨床抗菌藥物管理提供數(shù)據(jù)分析處理支持,提高臨床抗菌藥物管理水平,規(guī)范醫(yī)療行為,提高醫(yī)療質(zhì)量。
[Abstract]:Objective: the purpose of this project is to draw lessons from the experience of clinical antimicrobial management at home and abroad and to solve the problems existing in the management of domestic clinical antimicrobial agents. To study the effect of clinical antimicrobial drug management computer network system applied in clinic, to develop the software for inquiry and analysis of clinical antimicrobial drug use in hospital, and to use computer network, By analyzing the frequency of use of clinical antimicrobials and the proportion of antimicrobial prescriptions, the frequency of combined use of antimicrobial drugs was analyzed. To evaluate the management level of antimicrobial drugs in hospitals with the index of antimicrobial use intensity, so as to improve the management of antimicrobial drugs in hospitals, It is expected that the management level of antimicrobial agents will be improved significantly and the management level of antimicrobial agents in hospitals will be improved. Methods: through investigation, inquiry, follow up of clinical experts, doctors and pharmacists, to study the current situation and reasons of antimicrobial drug use, and to select the application of antimicrobial management system in a level II general hospital between July 2013 and June 2014 as the implementation group. At the same time, from July 2012 to June 2013, the control group was selected as the control group before the use of antimicrobial drug management system in the hospital from July 2012 to June 2013, and the two groups were compared. The statistical analysis was carried out by SPSS17.0 and Excel2007 statistical software. The measurement data were expressed as mean and standard deviation X2 鹵S, the comparison between the two groups was t test, and the count data was analyzed by 蠂 ~ 2 test. This study shows that, after the application of antimicrobial drug management system, The number of antibiotics prescriptions in the outpatient department of the implementation group was 6.43 lower than that in the control group, and the proportion of antibiotics used in the treatment group was 4.11% lower than that in the control group. The utilization rate of antibiotics in the group I was lower than that in the control group. The DDD value of the inpatients in the implementation group was 28.88 lower than that of the control group, and the P value was 0.05, which was statistically significant. The results suggest that there is a significant difference between the two groups. Conclusion: in this paper, the application background of antimicrobial management system and the value and method of antimicrobial management system are summarized and summarized. The main effect of monitoring antimicrobial drug management through the platform of practical application system is expounded. The computer network system of clinical antimicrobial management provides effective guidance and monitoring means for clinical use of antimicrobial agents. It has become the management of our hospital. The essential tools of clinical medicine work can provide data analysis and processing support for clinical antimicrobial drug management, improve the management level of clinical antimicrobial agents, standardize medical behavior and improve medical quality.
