【摘要】：目的了解醫(yī)院呼吸機(jī)管路污染和病原菌分布狀況,為提出呼吸機(jī)相關(guān)性肺炎防控措施的提供依據(jù)。方法對(duì)兩種不同配置的呼吸機(jī)管路進(jìn)行調(diào)查采樣,對(duì)管路內(nèi)不同部位進(jìn)行細(xì)菌總數(shù)檢測(cè)和細(xì)菌鑒定,并進(jìn)行比較。結(jié)果呼吸機(jī)管路越靠近患者端污染越重,兩種配置呼吸機(jī)管路使用7 d近患者端污染狀況有明顯差異,使用30 d的管路細(xì)菌總數(shù)明顯高于使用7 d的管路。濕化水污染嚴(yán)重,呼吸機(jī)送氣口污染較輕,但不使用細(xì)菌過濾器的回氣口有明顯污染。管路內(nèi)細(xì)菌種類鑒定次數(shù)排名前3的是銅綠假單胞菌、粘質(zhì)沙雷菌和洋蔥伯克霍爾德菌,兩醫(yī)院優(yōu)勢(shì)菌不同。結(jié)論國內(nèi)不同配置的呼吸機(jī)管路均存在明顯污染,尤其是呼吸機(jī)近患者端污染嚴(yán)重,對(duì)沒有氣道阻塞性疾病的患者,應(yīng)考慮使用細(xì)菌過濾型濕熱交換器替代加濕器;濕化水應(yīng)為無菌水并每天更換,并加強(qiáng)監(jiān)測(cè);應(yīng)加強(qiáng)患者呼吸道分泌物的檢驗(yàn)和近患者端管路污染的監(jiān)測(cè),設(shè)置適宜的換管周期。
[Abstract]:Objective to investigate the pollution of ventilator pipeline and the distribution of pathogenic bacteria in hospital, and to provide the basis for the prevention and control of ventilator-associated pneumonia. Methods two kinds of ventilator pipelines were investigated and sampled, and the total number of bacteria was detected and identified in different parts of the pipeline, and the results were compared. Results the closer the ventilator line was to the patient's end, the more serious the pollution was. There was significant difference in the pollution status between the two kinds of ventilator pipeline for 7 days. The total number of bacteria in the pipeline with 30 days was significantly higher than that in the 7 day pipeline. Humidification water pollution is serious, ventilator air supply port pollution is light, but the return port without bacterial filter has obvious pollution. Pseudomonas aeruginosa, Serratia vulgaris and Burkholderia onions were the top three bacteria in the pipeline, and the dominant bacteria in the two hospitals were different. Conclusion there is obvious pollution in different ventilator pipelines in China, especially in the near patient end of ventilator. For patients without airway obstruction, bacterial filter wet heat exchanger should be used to replace humidifier; humidified water should be replaced by aseptic water every day, and monitoring should be strengthened; the inspection of respiratory tract secretions and the monitoring of near patient end pipeline pollution should be strengthened, and the appropriate tube changing cycle should be set up.
