【摘要】： 摘要目的探討連云港農(nóng)村社區(qū)中老年高血壓患者動脈粥樣硬化的發(fā)生現(xiàn)狀及其與血壓、血糖、膽固醇和吸煙的關(guān)系。方法從2008年10月至2009年6月,采用流行病學(xué)現(xiàn)況調(diào)查方法,用區(qū)組隨機抽取連云港社區(qū)45～75歲原發(fā)性高血壓患者進行臨床流行病學(xué)調(diào)查,采用標(biāo)準操作規(guī)程進行血壓測量,全自動生化分析酶法檢測總膽固醇(total cholesterol, TC)。由專門接受培訓(xùn)的人員用同一臺彩色超聲儀器對對象的頸動脈內(nèi)膜-中膜厚度(CIMT)和斑塊進行檢查,由經(jīng)過統(tǒng)一培訓(xùn)的專門人員進行的問卷調(diào)查獲取流行病學(xué)資料。對各項指標(biāo)進行單因素和多因素分析。結(jié)果本研究共納入對象1346人,其中男585人,女761人。平均年齡男性59.88±7.65歲,女性58.59±7.32歲。CIMT平均厚度男性0.72±0.14mm,女性0.70±0.14mm。斑塊檢出率男性49.1%,女性32.5%。收縮壓平均水平男性167.91±20.84mmHg ,女性169.69±20.99mmHg。舒張壓平均水平男性98.02±12.27mmHg,女性94.56±12.23mmHg�？偰懝檀寄行�5.26±1.15mmol/L,女性5.47mmol/L。男性從不吸煙30.8%,現(xiàn)在吸煙51.7%。女性從不吸煙94.6%,現(xiàn)在吸煙2.9%。收縮壓與CIMT呈現(xiàn)正相關(guān),隨高血壓分級升高,檢出斑塊的風(fēng)險增加�？崭寡桥cCIMT之間不是線性關(guān)系。但空腹血糖≥7.0mmol/L的患者檢出2個以上斑塊的風(fēng)險顯著升高�？偰懝檀荚谀行匝芯繉ο笾信cCIMT呈現(xiàn)正相關(guān),總膽固醇≥5.69mmol/L時CIMT明顯增厚,檢出CIMT0.8mm的風(fēng)險顯著上升。而女性總膽固醇與CIMT無線性關(guān)系,但總膽固醇≥5.69mmol/L時,檢出患頸動脈斑塊的風(fēng)險顯著升高。CIMT與吸煙呈現(xiàn)正相關(guān),從不吸煙、曾經(jīng)吸煙和現(xiàn)在吸煙的對象CIMT厚度依次遞增。且現(xiàn)在吸煙的患者檢出斑塊的風(fēng)險升高。結(jié)論連云港社區(qū)中老年高血壓人群中有較高的動脈粥樣硬化患病率,其CIMT厚度與收縮壓呈正相關(guān)。空腹血糖超過7.0mmol/L或總膽固醇超過5.69mmol/L的高血壓患者發(fā)生動脈硬化的風(fēng)險增加。男性吸煙者的動脈硬化風(fēng)險亦增加。
[Abstract]:Objective to investigate the prevalence of atherosclerosis and its relationship with blood pressure, blood glucose, cholesterol and smoking in elderly hypertensive patients in Lianyungang rural community. Methods from October 2008 to June 2009, the patients with essential hypertension aged 4575 years old in Lianyungang community were randomly selected for clinical epidemiological investigation, and the blood pressure was measured by standard operating procedure. Detection of total cholesterol (total cholesterol, TC). By automatic biochemical analysis enzymatic method The carotid intima-media thickness (CIMT) and plaque were examined with the same color ultrasound instrument, and the epidemiological data were obtained by questionnaire. Single factor and multi-factor analysis were carried out on each index. Results A total of 1346 subjects were included in this study, including 585 males and 761 females. The mean age of CIMT was 59.88 鹵7.65 years old in male and 58.59 鹵7.32 years old in female. The mean thickness of CIMT was 0.72 鹵0.14 mm in male and 0.70 鹵0.14 mm in female. The plaque detection rate was 49.1% in men and 32.5% in women. The mean systolic blood pressure was 167.91 鹵20.84mmHg in males and 169.69 鹵20.99mm Hg in females. The mean diastolic blood pressure was 98.02 鹵12.27mmHg in males and 94.56 鹵12.23mmHg in females. Total cholesterol was 5.26 鹵1.15 mmol / L in men and 5.47 mmol / L in women. Men never smoke 30.8 and now smoke 51.7. Women never smoke 94. 6 and now smoke 2. 9. Systolic blood pressure (SBP) was positively correlated with CIMT, and the risk of plaque detection increased with hypertension grading.
