【摘要】：目的:探討蒼附導(dǎo)痰湯加減治療男性兒童乳房發(fā)育癥肝郁痰凝型的臨床療效;觀察蒼附導(dǎo)痰湯加減對(duì)患兒血清睪酮、雌二醇水平的影響。方法:選定符合納入標(biāo)準(zhǔn)的男性兒童乳房發(fā)育癥患者共60例,隨機(jī)分為治療組和對(duì)照組各30例。治療組給予口服中藥湯劑蒼附導(dǎo)痰湯加減,對(duì)照組給予口服逍遙丸。觀察周期為8周。8周后記錄兩組患者乳房癥狀積分和中醫(yī)癥候積分,進(jìn)行療效及安全性評(píng)估。檢測(cè)比較治療前后患兒血清睪酮、雌二醇水平。療程結(jié)束2個(gè)月后對(duì)兩組痊愈患兒進(jìn)行隨訪,記錄并對(duì)比復(fù)發(fā)率。結(jié)果:1.臨床總療效:治療組的總有效率為93.3%,對(duì)照組的總有效率為70.0%,治療組的總療效率明顯優(yōu)于對(duì)照組(P0.05)。2.乳房及中醫(yī)癥候積分比較:兩組患者治療前后在乳房疼痛、腫塊大小、腫塊硬度和中醫(yī)癥候四個(gè)方面積分的差異均具有統(tǒng)計(jì)學(xué)意義(P0.05);兩組患者治療后,組間差異同樣具有統(tǒng)計(jì)學(xué)意義(P0.05),說(shuō)明兩組治療均能改善患者乳房及中醫(yī)癥候情況,且治療組優(yōu)于對(duì)照組。激素水平比較:治療組患者的血清雌二醇和睪酮值在治療前后差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3.隨訪:治療組復(fù)發(fā)0例,對(duì)照組復(fù)發(fā)2例。治療組復(fù)發(fā)率明顯低于對(duì)照組(P0.05)。結(jié)論:1.蒼附導(dǎo)痰湯加減治療男童乳房發(fā)育癥肝郁痰凝型療效明顯優(yōu)于逍遙丸。2.蒼附導(dǎo)痰湯加減可改善乳房癥狀和中醫(yī)癥候;明顯提高血清睪酮水平,改善性激素紊亂狀態(tài)。3.蒼附導(dǎo)痰湯加減治療男童乳房發(fā)育癥安全、有效,無(wú)復(fù)發(fā)。
[Abstract]:Objective: to investigate the clinical effect of Cangfu Duan phlegm decoction (Cangfu Dantan decoction) on the serum testosterone and estradiol levels in male children with breast development syndrome with stagnation of liver and phlegm coagulation.Methods: 60 male children with breast development were randomly divided into treatment group (n = 30) and control group (n = 30).The treatment group was treated with oral Chinese medicine decoction Cangfu Daophan decoction and the control group with Xiaoyao Pill.The clinical efficacy and safety of the two groups were evaluated after the observation period of 8 weeks and 8 weeks after the breast symptom score and TCM symptom score were recorded.Serum testosterone and estradiol levels were measured before and after treatment.Two months after the course of treatment, the two groups were followed up and the recurrence rate was recorded and compared.The result is 1: 1.Total clinical efficacy: the total effective rate of the treatment group was 93.3 and the total effective rate of the control group was 70.00.The total effective rate of the treatment group was obviously better than that of the control group (P0.050.2.)Comparison of breast and TCM symptom scores: the differences of breast pain, mass size, mass hardness and TCM symptom between the two groups were statistically significant (P 0.05).The difference between the two groups was also statistically significant, indicating that the two groups can improve the breast and TCM symptoms, and the treatment group is better than the control group.Comparison of hormone levels: the serum estradiol and testosterone levels in the treatment group were significantly different before and after treatment.Follow up: there were 0 cases of recurrence in the treatment group and 2 cases in the control group.The recurrence rate in the treatment group was significantly lower than that in the control group (P 0.05).Conclusion 1.The therapeutic effect of Cangfu Daotan decoction in treating boy breast development syndrome with liver depression and phlegm coagulation is obviously better than Xiaoyao Pill. 2.Cangfu Daophan decoction can improve breast symptoms and TCM symptoms, significantly improve serum testosterone level, improve sex hormone disorder state. 3.Cangfu Daophan decoction is safe, effective and no recurrence in treating male breast development.
