注意缺陷多動(dòng)障礙兒童問(wèn)題行為及其影響因素分析

【摘要】:目的研究注意缺陷多動(dòng)障礙(ADHD)兒童問(wèn)題行為及其影響因素。方法于2013年10月—2014年6月,采用便利抽取法對(duì)黑龍江省某市選取5所公立小學(xué)1~4年級(jí)兒童進(jìn)行問(wèn)卷調(diào)查。采用一般情況調(diào)查表、Conners父母問(wèn)卷(PSQ)、兒童氣質(zhì)問(wèn)卷、Ruteer兒童行為問(wèn)卷(父母問(wèn)卷)、中文版兒童睡眠習(xí)慣問(wèn)卷(CSHQ)、父母養(yǎng)育方式量表進(jìn)行調(diào)查。根據(jù)PSQ多動(dòng)指數(shù)得分≥1.5分,判斷為ADHD初篩陽(yáng)性,并經(jīng)由ICD-10確診ADHD 132例,作為試驗(yàn)組,按照同質(zhì)性原則進(jìn)行匹配132例作為對(duì)照組。采用多因素Logistic回歸分析ADHD的影響因素。結(jié)果共發(fā)放問(wèn)卷6 280份,回收有效問(wèn)卷5 742份,有效回收率為91.4%。352例ADHD初篩陽(yáng)性,初篩陽(yáng)性率為6.1%,確診132例,患病率為2.3%。兩組氣質(zhì)問(wèn)卷中趨避性、情緒本質(zhì)得分比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);試驗(yàn)組氣質(zhì)問(wèn)卷中活動(dòng)度、規(guī)律性、適應(yīng)性、反應(yīng)強(qiáng)度、堅(jiān)持性、注意分散度、反應(yīng)閾得分高于對(duì)照組(P0.05)。對(duì)照組Ruteer兒童行為問(wèn)卷得分低于試驗(yàn)組(t=47.98,P0.001)。兩組CSHQ中睡眠焦慮、夜醒、異態(tài)睡眠、睡眠呼吸障礙、日間困倦、總分比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);試驗(yàn)組CSHQ中睡眠習(xí)慣、入睡潛伏期、睡眠持續(xù)時(shí)間得分低于對(duì)照組(P0.05)。對(duì)照組父親懲罰嚴(yán)厲、拒絕否認(rèn)、過(guò)度保護(hù)及母親過(guò)分干涉、拒絕否認(rèn)、懲罰嚴(yán)厲得分均低于試驗(yàn)組,母親情感溫暖得分高于試驗(yàn)組(P0.05)。多因素Logistic回歸分析結(jié)果顯示,出生體質(zhì)量、活動(dòng)度、趨避性、反應(yīng)強(qiáng)度、注意分散度、父親懲罰嚴(yán)厲、母親情感溫暖、母親懲罰嚴(yán)厲為ADHD的影響因素(P0.05)。結(jié)論 ADHD患兒存在較多的問(wèn)題行為,且存在家庭教養(yǎng)方式方面的危險(xiǎn)因素。
