【摘要】：目的既往研究表明,高危患者希望享受高技術(shù)含量的協(xié)調(diào)初級(jí)醫(yī)療。然而,醫(yī)生對(duì)于低收入、慢性疾病患者和為其提供護(hù)理的人員在初級(jí)醫(yī)療過(guò)程各個(gè)步驟中的體驗(yàn)了解甚少。方法納入未參加保險(xiǎn)或醫(yī)療補(bǔ)助計(jì)劃的慢性疾病患者和初級(jí)醫(yī)療人員,對(duì)其進(jìn)行定性訪談。對(duì)21名患者和來(lái)自3家初級(jí)醫(yī)療機(jī)構(gòu)(包括1家聯(lián)邦資格衛(wèi)生院和兩家學(xué)術(shù)附屬診所)各個(gè)崗位的30名員工進(jìn)行訪談。結(jié)果訪談內(nèi)容為貫穿初級(jí)醫(yī)療所有階段的3個(gè)主要問(wèn)題:(1)盡管建立了促進(jìn)交流的系統(tǒng),但每次治療的信息交流依舊存在困難;(2)患者、臨床醫(yī)生和員工的目標(biāo)和期待不一致,為每次治療服務(wù)的提供和獲取帶來(lái)障礙;(3)患者和員工高度重視人際關(guān)系。結(jié)論弱勢(shì)人群和初級(jí)醫(yī)療人員在整個(gè)初級(jí)醫(yī)療過(guò)程中共同經(jīng)歷著一些困難。促進(jìn)信息交流、建立一致的目標(biāo)和期待、增進(jìn)人際關(guān)系可以提升患者和員工的醫(yī)療體驗(yàn)。
[Abstract]:Objective previous studies have shown that high-risk patients want to benefit from highly skilled coordinated primary care. However, doctors have low incomes. There is little understanding of the experiences of patients with chronic diseases and those who provide them with care in the various steps of the primary medical process. Methods include patients with chronic diseases and primary health care personnel who are not covered by insurance or Medicaid schemes. Qualitative interviews. 21 patients and 3 primary health care institutions (including 1 federal qualification hospital and 2 academic affiliated clinics). Interviews were conducted among 30 employees in various positions. Results the interview covered three major questions: 1, which ran through all stages of primary health care, despite the establishment of a system to promote communication. However, the information exchange of each treatment is still difficult; (2) inconsistent goals and expectations of patients, clinicians and employees present obstacles to the provision and access of services for each treatment; Conclusion the disadvantaged group and the primary medical staff are experiencing some difficulties in the whole process of primary medical care, and promote the exchange of information and establish a consistent goal and expectation. 3) patients and employees attach great importance to interpersonal relationships. Improving relationships can enhance the medical experience of patients and employees.
