【摘要】：目的:探索從濕辨證論治糖尿病性干眼癥的中醫(yī)理論及其治療原則,通過觀察祛濕化濁、理氣運脾法治療濕濁中阻、氣機不暢型糖尿病干眼癥的臨床效果,并通過臨床案例分析,豐富中醫(yī)對本病的認識,豐富中醫(yī)治療糖尿病干眼癥的治療方法和及治療原則。方法:將收集到的糖尿病合并干眼癥患者病例隨機分為兩組,治療組30例,對照組30例。治療組采用祛濕化濁、理氣運脾法聯(lián)合愛麗滴眼液治療,對照組僅用愛麗滴眼液治療,兩組均以1個月為一個療程,治療兩個療程后觀察治療前以及治療后患者雙眼的主觀癥狀(眼干澀、眼紅、眼癢、異物感、視物疲勞、眼疼痛),以及眼部體征(淚液分泌量、淚膜破裂的時間、角膜染色的情況、角膜知覺),并進行療效評估。結(jié)果:顯示治療組顯效11例,有效15例,進步4例,無效0例;對照組顯效0例,有效3例,進步27例,無效0例。治療組各項觀察指標都有明顯改善,差異具有統(tǒng)計學意義(P0.05);對照組患者中眼紅、眼癢、Schirmer I Test無統(tǒng)計學意義(P0.05),其它癥狀及體征差異均有統(tǒng)計學意義(P0.05)。應(yīng)用祛濕化濁、理氣運脾法可改善糖尿病干眼癥濕濁中阻型患者的主觀癥狀,增加淚液分泌量,還可以改善角膜情況。結(jié)論:祛濕化濁、理氣運脾法可以明顯改善糖尿病干眼癥患者癥狀,療效明顯。從濕論治糖尿病干眼癥可以為此病的中醫(yī)診治提供新方法,新思路。
[Abstract]:Objective: to explore the theory of traditional Chinese medicine (TCM) and its principle of treating diabetic xerophthalmia from the view of dampness differentiation and syndrome differentiation, and to observe the clinical effect of removing dampness and removing turbidity and regulating qi and regulating spleen in treating diabetic dry eye disease of type of dampness and stagnation of qi. Through the analysis of clinical cases, enrich the understanding of TCM to this disease, enrich the traditional Chinese medicine treatment of diabetes mellitus dry eye treatment methods and treatment principles. Methods: the patients with diabetes mellitus complicated with dry eye were randomly divided into two groups: treatment group (n = 30) and control group (n = 30). The treatment group was treated with removing dampness and removing turbidity, regulating qi and regulating spleen combined with Allie eye drops, while the control group was treated only with Ally eye drops, and both groups were treated with one month as a course of treatment. After two courses of treatment, the subjective symptoms of both eyes (dry eyes, red eyes, itchy eyes, foreign matter sensation, visual fatigue, eye pain), as well as eye signs (the amount of lacrimal secretion, the time of tear film rupture), were observed before and after treatment. Corneal staining, corneal perception) and evaluation of the efficacy. Results: there were 11 cases in the treatment group, 15 cases in the effective group, 4 cases in the improvement, 0 cases in the control group, 0 cases in the control group, 3 cases in the control group, 27 cases in the improvement and 0 cases in the control group. The observation indexes of the treatment group were obviously improved, the difference was statistically significant (P0.05). Control group patients with red eyes, itching, Schirmer I Test had no statistical significance (P0.05), other symptoms and signs were statistically significant differences (P0.05). The method of removing dampness and removing turbidity and regulating qi and regulating spleen can improve the subjective symptoms, increase the amount of tear secretion, and improve the corneal condition in patients with diabetic dry eye disease with dampness and turbidity. Conclusion: the method of removing dampness and removing turbidity and regulating qi and regulating spleen can obviously improve the symptoms of diabetic dry eye patients, and the curative effect is obvious. The treatment of diabetes dry eye from the view of dampness can provide new methods and new ideas for the diagnosis and treatment of this disease.
