【摘要】：目的:總結(jié)導(dǎo)師張曉杰教授從“肝”論治尋常型銀屑病的臨床經(jīng)驗(yàn),觀察導(dǎo)師經(jīng)驗(yàn)方治療尋常型銀屑病的的臨床療效,探索中醫(yī)辨證治療尋常型銀屑病新的思路和方法。方法:收集山東省中醫(yī)院皮膚科門診80例尋常型銀屑病患者,將其辨證分型,分為肝經(jīng)血熱型、肝經(jīng)濕熱型、肝經(jīng)瘀熱型、肝虛血燥型,分別給予清肝涼血湯、加味龍膽瀉肝湯、郁金消銀湯、潤燥消銀湯治療,外用凡士林軟膏和龍珠軟膏,2周為1療程,觀察8周。在觀察開始時和結(jié)束時填寫臨床觀察表和情緒自評量表,根據(jù)治療前后患者皮損PASI評分分析療效,通過情緒自評量表分析尋常型銀屑病與情緒因素的相關(guān)性,通過記錄患者的不良反應(yīng)來監(jiān)測藥物副作用。結(jié)果:所有患者治療后,總愈顯率為72.5%,總有效率為97.5%。所有患者治療前PASI評分均值為(17.82±5.23),治療后PASI評分均值為(5.66±6.26),治療前后PASI評分比較,差異具有統(tǒng)計學(xué)意義(P㩳0.01)。所有患者治療前SAS標(biāo)準(zhǔn)分均值為(53.28±10.39),SDS標(biāo)準(zhǔn)分均值為(56.61±11.77);治療后SAS標(biāo)準(zhǔn)分均值為(42.34±11.77),SDS標(biāo)準(zhǔn)分均值為(42.35±13.53),治療后較治療前分值降低(P㩳0.01),差異具有統(tǒng)計學(xué)意義。結(jié)論:導(dǎo)師從“肝”辨證分型論治尋常型銀屑病臨床療效確切,導(dǎo)師經(jīng)驗(yàn)方治療尋常型銀屑病有效率較高、副作用較少。焦慮、抑郁等情緒因素對尋常型銀屑病的發(fā)生發(fā)展有一定作用,導(dǎo)師經(jīng)驗(yàn)方可降低部分銀屑病患者的焦慮、抑郁評分。
[Abstract]:Objective: to summarize the clinical experience of professor Zhang Xiaojie in treating psoriasis vulgaris from the viewpoint of "liver", observe the clinical effect of the prescription for treating psoriasis vulgaris, and explore the new thinking and method of treating psoriasis vulgaris based on TCM syndrome differentiation. Methods: 80 patients with psoriasis vulgaris in dermatology department of Shandong traditional Chinese Medicine Hospital were divided into three types: liver meridian blood heat type, liver meridian damp-heat type, liver meridian stasis heat type, liver deficiency and blood dryness type, respectively. Modified Longdan Xiegan decoction, Yujin Xiaoyin decoction, Runzao Xiaoyin decoction treatment, external use of vaseline ointment and Longzhu ointment, 2 weeks as a course of treatment, observe 8 weeks. At the beginning and the end of the observation, the clinical observation form and the emotional self-rating scale were filled out. According to the PASI score of skin lesions of the patients before and after treatment, the correlation between psoriasis vulgaris and emotional factors was analyzed by self-rating emotional scale. Adverse drug side effects were monitored by recording adverse reactions in patients. Results: after treatment, the total recovery rate was 72.5% and the total effective rate was 97.5%. The mean PASI score before and after treatment was (17.82 鹵5.23) and (5.66 鹵6.26) respectively. The difference of PASI score before and after treatment was statistically significant (P0.01). The mean SAS standard score before treatment was (53.28 鹵10.39), SDS (56.61 鹵11.77). The mean standard score of SAS was (42.34 鹵11.77), SDS) after treatment (42.35 鹵13.53), which was significantly lower than that before treatment (P0. 01). Conclusion: according to the syndrome differentiation and classification of "liver", the tutor is effective in treating psoriasis vulgaris, and the effective rate is higher and the side effect is less. Anxiety, depression and other emotional factors have a certain effect on the occurrence and development of psoriasis vulgaris. Mentors can reduce the anxiety and depression scores of some psoriasis patients.
