【摘要】：目的:觀察補(bǔ)腎養(yǎng)血膏方在肱骨外科頸骨折切開(kāi)復(fù)位鋼板內(nèi)固定術(shù)后治療的臨床療效。方法:本觀察病例均來(lái)自廣西中醫(yī)藥大學(xué)附屬瑞康醫(yī)院2014年1月~2015年1月間行住院手術(shù)治療的肱骨外科頸骨折患者60例,患者隨機(jī)分為觀察組30例和對(duì)照組30例。觀察組采用切開(kāi)復(fù)位鋼板內(nèi)固定術(shù)配合早期服用補(bǔ)腎養(yǎng)血膏方治療;對(duì)照組采用切開(kāi)復(fù)位鋼板內(nèi)固定術(shù)治療。術(shù)后兩組都進(jìn)行肩關(guān)節(jié)功能鍛煉。比較兩組肩關(guān)節(jié)功能恢復(fù)情況及臨床治療效果。結(jié)果:所有患者均獲6~12個(gè)月的隨訪(平均隨訪8個(gè)月)。所有患者沒(méi)有出現(xiàn)術(shù)后感染、內(nèi)固定物松動(dòng)、移位或者斷裂等并發(fā)癥。觀察組與對(duì)照組在Neer肩關(guān)節(jié)功能評(píng)分及臨床效果等方面的比較具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)果按Neer肩關(guān)節(jié)評(píng)分系統(tǒng)對(duì)兩組患者術(shù)后肩關(guān)節(jié)功能進(jìn)行評(píng)分,觀察組優(yōu)25例,良4例,可1例,差0例,優(yōu)良率96.7%;對(duì)照組優(yōu)18例,良7例,可3例,差2例,優(yōu)良率83.3%;兩組患者臨床療效比較,觀察組治愈21例,好轉(zhuǎn)9例,未愈0例;對(duì)照組治愈13例,好轉(zhuǎn)17例,未愈0例。結(jié)論:補(bǔ)腎養(yǎng)血膏方在肱骨外科頸骨折術(shù)后使用,具有改善局部血液循環(huán),促進(jìn)骨折愈合,肩關(guān)節(jié)功能恢復(fù)滿意等優(yōu)點(diǎn)。
[Abstract]:Objective: to observe the clinical effect of Bushen Yangxue ointment on open reduction and internal fixation of humeral surgical neck fracture. Methods: all the 60 cases of humeral surgical neck fracture treated by operation from January 2014 to January 2015 in Ruikang Hospital affiliated to Guangxi University of traditional Chinese Medicine were randomly divided into observation group (n = 30) and control group (n = 30). The observation group was treated with open reduction plate internal fixation and early treatment with Bushen Yangxue ointment, while the control group was treated with open reduction plate internal fixation. The function of shoulder joint was performed in both groups after operation. To compare the recovery of shoulder joint function and the effect of clinical treatment between the two groups. Results: all patients were followed up for 6 ~ 12 months (mean 8 months). No postoperative infection, internal fixation loosening, displacement or rupture were found in all patients. Observation group and control group in Neer shoulder function score and clinical effect were statistically significant (P0.05). Results according to Neer's shoulder joint scoring system, 25 cases of excellent, 4 cases of good, 1 case of fair, 0 case of poor, excellent and good rate of 96.7in the observation group, 18 cases of excellent, 7 cases of good, 3 cases of fair, 2 cases of poor in the control group. There were 21 cases cured, 9 cases improved and 0 cases uncured in the observation group, 13 cases were cured, 17 cases improved, and 0 cases were not recovered in the control group. Conclusion: the application of Bushen Nourishing Blood Ointment in humeral surgical neck fracture has the advantages of improving local blood circulation, promoting fracture healing, and satisfactory recovery of shoulder joint function.
