【摘要】：目的：通過(guò)火針密刺治療慢性濕疹的療效觀察,探究火針作用機(jī)理,為火針治療慢性濕疹提供臨床依據(jù),。方法：本研究通過(guò)收集在2015年2月至2016年2月時(shí)間段在廣州中醫(yī)藥大學(xué)第一附屬醫(yī)院針灸科和皮膚科就診的符合慢性濕疹診斷標(biāo)準(zhǔn)和課題納入標(biāo)準(zhǔn)的60例患者作為研究對(duì)象,隨機(jī)分配治療組30例和對(duì)照組30例。治療組予火針密刺濕疹皮損部位,1周治療3次；對(duì)照組根據(jù)濕疹部位予外涂鹵米松乳膏,1天1次,兩組療程均為3周。3周后觀察兩組濕疹皮損情況,根據(jù)濕疹面積及嚴(yán)重度指數(shù)(Eczema area and severity index,簡(jiǎn)稱EASI)量表統(tǒng)計(jì)病例治療前后情況數(shù)據(jù)。在治療結(jié)束一個(gè)月后電話隨訪痊愈患者,收集兩種療法的復(fù)發(fā)情況。通過(guò)近期療效、長(zhǎng)期療效、復(fù)發(fā)情況三方面指標(biāo)去評(píng)價(jià)火針治療慢性濕疹的臨床療效。成果：治療組30例與對(duì)照組30例完成本研究試驗(yàn),兩組患者在年齡、病程、皮損嚴(yán)重程度的差異無(wú)統(tǒng)計(jì)學(xué)意義(p0.05),兩組在性別上的差異無(wú)統(tǒng)計(jì)學(xué)意義(x2=0.287,p=0.789),即兩組慢性濕疹患者具有可比性。EASI積分評(píng)價(jià)上,治療組治療前EASI積分13.89±5.00,治療后積分3.22±3.05；對(duì)照組治療前EASI積分13.65±5.59,治療后積分5.66±5.27,組內(nèi)治療前后療效經(jīng)配對(duì)樣本t檢驗(yàn),均p0.01,治療前后療效有明顯統(tǒng)計(jì)學(xué)意義,說(shuō)明火針和鹵米松乳膏治療慢性濕疹都有顯著療效。而治療組治療后積分與對(duì)照組比較,經(jīng)獨(dú)立樣本t檢驗(yàn),p=0.033(p0.05),說(shuō)兩組療效有統(tǒng)計(jì)學(xué)差異,火針治療慢性濕疹療效優(yōu)于鹵米松乳膏。療效評(píng)價(jià)上,治療組痊愈8例,顯效12例,有效7例,無(wú)效3例,總好轉(zhuǎn)率為90%；對(duì)照組中痊愈5例,顯效13例,有效6例,無(wú)效6例,總好轉(zhuǎn)率為80%。治療組與觀察組療效經(jīng)過(guò)秩和檢驗(yàn)得：p=0.030(p0.05),說(shuō)明兩組療效差異有統(tǒng)計(jì)學(xué)意義,即火針治療慢性濕疹療效優(yōu)于鹵米松乳膏。在療程結(jié)束后一個(gè)月電話隨訪中,治療組痊愈8例中無(wú)復(fù)發(fā)情況,對(duì)照組痊愈5例中有2例復(fù)發(fā),可知火針對(duì)于慢性濕疹患者的長(zhǎng)期療效優(yōu)于鹵米松乳膏。結(jié)論：治療組火針治療慢性濕疹療效優(yōu)于對(duì)照組鹵米松乳膏,能明顯減輕患者濕疹的病情,可為慢性濕疹患者多提供一種治療手段,該療法值得臨床上加以推廣。
[Abstract]:Objective: to investigate the mechanism of fire acupuncture in the treatment of chronic eczema and to provide clinical basis for fire acupuncture treatment of chronic eczema. Methods: from February 2015 to February 2016, 60 patients with chronic eczema who met the diagnostic criteria and subject inclusion criteria in the Department of Acupuncture and Dermatology, first affiliated Hospital of Guangzhou University of traditional Chinese Medicine, were collected as subjects. 30 cases in the treatment group and 30 cases in the control group were randomly assigned. In the treatment group, the skin lesions of the eczema were treated with fire acupuncture for 3 times a week, and the patients in the control group were treated with halogen milisone cream once a day according to the site of eczema. The course of treatment in both groups was 3 weeks and 3 weeks later, the lesions of eczema were observed in both groups. According to eczema area and severity index (EASI), the data of cases before and after treatment were analyzed. A month after the end of treatment, the patients were followed up by telephone and the recurrence of both treatments was collected. To evaluate the clinical efficacy of fire acupuncture in treating chronic eczema by three indexes: short-term curative effect, long-term curative effect and recurrence. Results: 30 cases in the treatment group and 30 cases in the control group completed the study. There was no significant difference in the severity of skin lesions between the two groups (p0.05), and there was no significant difference in sex between the two groups (x20.287, p0.789). That is to say, the scores of EASI before and after treatment were 13.89 鹵5.00 before treatment and 3.22 鹵3.05 after treatment in the two groups. The scores of EASI before and after treatment in the control group were 13.65 鹵5.59 and 5.66 鹵5.27 respectively. After treatment, the curative effect of the control group was tested by paired sample t test (p0.01). The curative effect was statistically significant before and after treatment, indicating that fire needle and halomethasone cream had significant curative effect on chronic eczema. Compared with the control group, the therapeutic effect of the treatment group was better than that of halomethasone cream after treatment, and the curative effect was statistically different between the two groups by independent t-test (p0.05), and the curative effect of the two groups was better than that of halomethasone cream in the treatment of chronic eczema. In the treatment group, 8 cases were cured, 12 cases were effective, 7 cases were effective, 3 cases were ineffective, the total improvement rate was 90%, while in the control group, 5 cases were cured, 13 cases were markedly effective, 6 cases were ineffective, and the total improvement rate was 80%. The curative effect of the treatment group and the observation group was obtained by rank sum test. The difference between the two groups was statistically significant, that is, the curative effect of fire acupuncture on chronic eczema was better than that of halomethasone cream. In the telephone follow-up of one month after the end of the treatment, there was no recurrence in 8 cases in the treatment group and 2 cases in the 5 cases in the control group. The long-term curative effect of fire on chronic eczema patients was better than that of halomethasone cream. Conclusion: the therapeutic effect of fire acupuncture in treatment group is better than halomethasone cream in control group, it can obviously alleviate the condition of eczema, and can provide a more therapeutic method for chronic eczema patients. This therapy is worth popularizing in clinic.
