【摘要】：目的：觀察黃術(shù)合劑配合肛周亞甲藍(lán)注射對(duì)濕熱下注型肛門瘙癢癥臨床療效。方法：將2015年2月～2016年1月符合納入標(biāo)準(zhǔn)的60例肛門瘙癢癥患者通過(guò)簡(jiǎn)單隨機(jī)分組分為治療組、對(duì)照組各30例,兩組病例術(shù)前、術(shù)后均采用相同的基礎(chǔ)治療,治療組術(shù)后予口服黃術(shù)合劑；術(shù)后第10天、2個(gè)月分別觀察記錄兩組患者肛門瘙癢、潮濕、疼痛、皮損癥狀。結(jié)果：完成研究的病例共60例,對(duì)照組(N=30)和治療組(N=30)。治療前對(duì)兩組病例的性別、年齡、病程及治療前一般情況比較、瘙癢潮濕疼痛皮損面積的各項(xiàng)積分比較評(píng)分等進(jìn)行比較,差異均無(wú)統(tǒng)計(jì)學(xué)意義,具有可比性。研究結(jié)果如下：(1)兩組治療前和術(shù)后第10天在瘙癢、潮濕、疼痛及皮損面積項(xiàng)積分的比較,及兩組治療前和術(shù)后2個(gè)月在瘙癢、潮濕、疼痛及皮損面積項(xiàng)積分的比較,兩組在術(shù)后第10天及術(shù)后2個(gè)月各項(xiàng)積分均少于治療前各項(xiàng)積分,說(shuō)明在近期療效和遠(yuǎn)期療效中治療組和對(duì)照組均能改善瘙癢、潮濕、疼痛、皮損面積；(2)兩組在術(shù)后第10天(近期療效)疼痛項(xiàng)積分的比較及兩組術(shù)后2個(gè)月(遠(yuǎn)期療效)疼痛項(xiàng)積分的比較,經(jīng)秩和檢驗(yàn),兩組在術(shù)后第10天及術(shù)后2個(gè)月疼痛項(xiàng)積分P均0.05,差異無(wú)統(tǒng)計(jì)學(xué)意義；說(shuō)明在近期療效和遠(yuǎn)期療效中治療組在減少搔抓后而引起的肛門疼痛的積分方面療效未見(jiàn)明顯優(yōu)于對(duì)照組；(3)兩組在術(shù)后第10天(近期療效)瘙癢、潮濕及皮損面積項(xiàng)積分的比較及術(shù)后2個(gè)月(遠(yuǎn)期療效)瘙癢、潮濕及皮損面積項(xiàng)積分的比較,經(jīng)秩和檢驗(yàn),兩組在術(shù)后第10天及術(shù)后2個(gè)月瘙癢、潮濕、皮損面積項(xiàng)積分P均0.05,說(shuō)明治療組在近期療效和遠(yuǎn)期療效中減少肛門瘙癢、潮濕、皮損面積積分方面療效明顯優(yōu)于對(duì)照組,即治療組在減輕肛門瘙癢、潮濕癥狀及恢復(fù)皮損的療效方面優(yōu)于對(duì)照組。(4)兩組手術(shù)后第10天和術(shù)后2個(gè)月的總體療效比較,P均0.05,均有統(tǒng)計(jì)學(xué)意義,說(shuō)明在近期療效和遠(yuǎn)期療效中治療組總體療效均明顯優(yōu)于對(duì)照組；治療組近期總有效率83.33%,對(duì)照組76.67%,治療組遠(yuǎn)期總有效率83.33%,對(duì)照組73.33%,用統(tǒng)計(jì)學(xué)方法處理,兩組總療效比較有顯著性差異(P0.05)；兩組間癥狀療效比較,在治療肛門瘙癢方面存在顯著性差異(P0.05),治療組優(yōu)于對(duì)照組。結(jié)論：黃術(shù)合劑具有清熱利濕止癢之功,黃術(shù)合劑配合肛周亞甲藍(lán)注射能明顯改善肛門瘙癢癥的瘙癢、潮濕癥狀,對(duì)濕熱下注型肛門瘙癢癥有良好的治療作用。
[Abstract]:Objective: to observe the clinical effect of Huangzhu mixture combined with perianal methylene blue injection in treating anal pruritus with damp-heat injection. Methods: from February 2015 to January 2016, 60 patients with anal pruritus were randomly divided into treatment group (n = 30) and control group (n = 30). The patients in both groups were treated with the same basic treatment before and after operation. The patients in the treatment group were treated with Huangzhu mixture 10 days and 2 months after operation, and the symptoms of anal itching, damp, pain and skin lesions were observed and recorded in the two groups. Results: 60 cases (control group) and treatment group (n = 30) completed the study. Before treatment, the gender, age, course of disease and general situation before treatment were compared between the two groups, and the scores of each score of skin area of pruritus and moist pain were compared. The differences were not statistically significant and comparable. The results were as follows: (1) the scores of pruritus, humidity, pain and area of skin lesions were compared between the two groups before and 10 days after treatment, and the scores of itching, damp, pain and area of skin lesions were compared between the two groups before and 2 months after treatment. The scores of both groups on the 10th day after operation and 2 months after operation were less than those before treatment, which indicated that both the treatment group and the control group could improve the area of pruritus, damp, pain and skin lesion in the short-term and long-term curative effect. (2) the scores of pain items in the two groups were compared on the 10th day after operation (short-term curative effect) and 2 months after operation (long-term curative effect), the scores of pain items were compared by rank sum test. There was no significant difference in pain score between the two groups (P < 0.05) on the 10th day after operation and 2 months after operation. The results showed that the curative effect of the treatment group was not significantly better than that of the control group in reducing the score of anal pain caused by scratching in the short and long term.) the pruritus of the treatment group was not significantly better than that of the control group on the 10th day after operation (short-term curative effect). The comparison of the score of wet area and skin lesion area and the score of itching at 2 months after operation, the score of wet area and area of skin lesion were compared by rank sum test, the two groups had itching and damp on the 10th day after operation and 2 months after operation. The score of the area of skin lesion was 0.05, which indicated that the treatment group was superior to the control group in reducing anal pruritus and damp in the short-term and long-term curative effect, that is, the treatment group was better than the control group in reducing the anal itching. The effect of wet symptoms and recovery of skin lesions was better than that of control group (P < 0.05), 10 days after operation and 2 months after operation. The results showed that the overall curative effect of the treatment group was obviously better than that of the control group in both the short-term and the long-term curative effect, the total effective rate in the treatment group was 83.33, the control group was 76.67, the total long-term effective rate in the treatment group was 83.33, the control group was 73.33, and the statistical method was used. There was significant difference in the total curative effect between the two groups (P 0.05), and there was significant difference in the treatment of anal pruritus between the two groups (P 0.05), and the treatment group was superior to the control group. Conclusion: Huangzhu mixture has the function of clearing heat, promoting dampness and stopping itching. Huangzhu mixture combined with perianal methylene blue injection can obviously improve the pruritus and damp symptoms of anal pruritus, and has a good therapeutic effect on the anal pruritus caused by damp-heat injection.
