【摘要】：目的:通過(guò)觀察柏硝洗劑對(duì)創(chuàng)面濕敷在促進(jìn)肛瘺術(shù)后愈合過(guò)程中的創(chuàng)面疼痛、水腫程度、創(chuàng)面滲液程度、顯微鏡下觀察創(chuàng)面新生肉芽組織的成纖維細(xì)胞數(shù)量和創(chuàng)面愈合時(shí)間這幾項(xiàng)指標(biāo),探討柏硝洗劑加速創(chuàng)面愈合的機(jī)理,從而對(duì)柏硝洗劑的臨床應(yīng)用提出科學(xué)、可靠的理論依據(jù)。方法:將60例符合本研究入選標(biāo)準(zhǔn)的低位單純性肛瘺術(shù)后病人,隨機(jī)地分成治療組及對(duì)照組各30例,兩組患者在常規(guī)換藥和紅光照射的基礎(chǔ)上,從術(shù)后第一天起治療組使用柏硝洗劑50ml用隔袋溫?zé)岷髮⒁粔K醫(yī)用紗布浸濕后放至創(chuàng)面進(jìn)行濕敷,每次濕敷20min,早晚各一次;而對(duì)照組采用金玄痔科熏洗散一袋,加1000ml開水化開后,趁熱先熏蒸肛門,再坐浴,每次20分鐘,早晚各一次。觀察兩組病人的創(chuàng)面愈合時(shí)間及術(shù)后3、7、12天兩組患者間疼痛、水腫程度、創(chuàng)面滲液程度和術(shù)后第7、12天顯微鏡下觀察創(chuàng)面新生肉芽組織的成纖維細(xì)胞數(shù)量的差別。結(jié)果:柏硝洗劑在創(chuàng)面愈合時(shí)間方面與對(duì)照組相比較平均約提前3天,有顯著差異(P0.01)。同時(shí),柏硝洗劑在減輕術(shù)后創(chuàng)面疼痛、水腫、滲液及提高創(chuàng)面肉芽組織的成纖維細(xì)胞數(shù)量方面仍較對(duì)照組有顯著優(yōu)勢(shì)(P0.05)。結(jié)論:經(jīng)觀察研究,柏硝洗劑具有清熱燥濕、解毒消腫療瘡的臨床療效,通過(guò)使用柏硝洗劑對(duì)術(shù)后的創(chuàng)面進(jìn)行濕敷可以明顯緩解術(shù)后患者的創(chuàng)面疼痛、水腫程度及創(chuàng)面滲液的現(xiàn)象,能加速術(shù)后創(chuàng)面組織的成纖維細(xì)胞增殖,進(jìn)而可以減少患者創(chuàng)面的愈合時(shí)間,可作為加速肛瘺術(shù)后創(chuàng)面愈合的有效方法,無(wú)明顯不良反應(yīng)。
[Abstract]:Objective: to observe the degree of wound pain, edema and exudation in the process of promoting the healing of anal fistula by wet application of Burnite lotion. The number of fibroblasts and wound healing time of new granulation tissue were observed under microscope, and the mechanism of Burberry lotion to accelerate wound healing was discussed. The scientific and reliable theoretical basis for clinical application of Burnite lotion was put forward. Methods: sixty patients with low simple anal fistula were randomly divided into treatment group (n = 30) and control group (n = 30). From the first day after operation, the treatment group was treated with 50ml, then soaked a piece of medical gauze and put it into the wound for 20 minutes each time, once in the morning and evening, while the control group was treated with Jinxuan Zhike fumigation and washing powder, and then opened with 1000ml boiling water. Fumigation anus while hot, sit bath again, 20 minutes each time, morning and evening each time. The time of wound healing and the degree of pain, edema, exudation of wound surface and the number of fibroblasts of new granulation tissue were observed under microscope on the 712th day after operation. Results: compared with the control group, the healing time of Burnite lotion was 3 days earlier than that of the control group (P 0.01). At the same time, P0. 05 lotion still had a significant advantage over the control group in relieving pain, edema, exudation and increasing the number of fibroblasts in granulation tissue after operation. Conclusion: through observation and study, the Burnite lotion has the clinical curative effect of clearing heat, dryness and dampness, detoxification and detumescence, and can relieve the pain of the wounds after operation by wet application of Burnite lotion. The degree of edema and the phenomenon of wound exudation can accelerate the proliferation of fibroblasts in wound tissue and reduce the wound healing time. It can be used as an effective method to accelerate the wound healing after anal fistula operation without obvious adverse reactions.
