【摘要】：目的:探討龍膽瀉肝湯聯(lián)合刺血療法治療老年帶狀皰疹后遺神經(jīng)痛的臨床療效。方法:選取研究對象64例老年帶狀皰疹后遺神經(jīng)痛患者,隨機分成兩組,每組32例。對照組給予常規(guī)綜合治療,維生素B1+甲鈷胺片+加巴噴丁膠囊,治療組在此基礎(chǔ)上再予龍膽瀉肝湯聯(lián)合刺血療法治療,連續(xù)治療15 d。檢測治療前后外周血T淋巴細(xì)胞亞群及血清P物質(zhì)水平,VAS、QOL評分,觀察其臨床療效及安全性。結(jié)果:與治療前比較,兩組外周血CD_3~+、CD_4~+以及CD_4~+/CD_8~+明顯升高(P0.05),CD_8~+明顯降低(P0.05),血清P物質(zhì)水平以及VAS評分顯著降低(P0.01),QOL評分顯著升高(P0.01);與對照組比較,治療組外周血CD_4~+以及CD_4~+/CD_8~+較高(P0.05),CD_8~+較低(P0.05),血清P物質(zhì)水平以及VAS評分較低(P0.01),QOL評分較高(P0.01),治療有效率較高(P0.05)。結(jié)論:龍膽瀉肝湯聯(lián)合刺血療法能顯著減輕老年帶狀皰疹后遺神經(jīng)痛患者的疼痛癥狀,提高生存質(zhì)量,可能機制為調(diào)節(jié)T淋巴細(xì)胞亞群及降低血清P物質(zhì)水平。
[Abstract]:Objective: to investigate the clinical effect of Longdan Xiegan decoction combined with blood pricking therapy on senile postherpetic neuralgia. Methods: 64 elderly patients with postherpetic herpes zoster neuralgia were randomly divided into two groups. 32 cases in each group. The control group was treated with routine comprehensive treatment, vitamin B1 methylcobalamin tablets plus bapentin capsule, and the treatment group was treated with Longdan Xiegan decoction combined with blood pricking therapy on this basis. After 15 days of continuous treatment, the peripheral blood T lymphocyte subsets and serum substance P level were measured and VAS-QOL scores were measured to observe the clinical efficacy and safety. Results: compared with before treatment. The levels of CD3 / CD8 in peripheral blood of both groups were significantly higher than those of CD3 / CD8 / CDS. Serum substance P level and VAS score decreased significantly (P 0.01) and QOL score increased significantly (P 0.01). Compared with the control group, the peripheral blood CD4 ~ and CD4 ~ / CD8 / CDT in the treatment group were higher than that in the control group (P0.05 ~ lower P0.05). The serum level of substance P and the VAS score were lower (P 0.01) and higher (P 0.01). Conclusion: Gentian Xiegan decoction combined with blood pricking therapy can significantly reduce the pain symptoms of the elderly patients with postherpetic neuralgia and improve the quality of life. The possible mechanism is to regulate T lymphocyte subsets and reduce the level of substance P in serum.
