甲狀腺和甲狀旁腺腫瘤內鏡輔助及內鏡手術治療

【摘要】：甲狀腺和甲狀旁腺腫瘤是頭頸部常見的腫瘤之一,手術是其主要治療方法。隨著微創(chuàng)外科和微創(chuàng)思潮的發(fā)展及患者對美容要求的提高,微創(chuàng)外科的技術及觀念已滲透到甲狀(旁)腺外科領域,內鏡甲狀(旁)腺切除術應運而生。與傳統(tǒng)開放甲狀腺手術相比內鏡甲狀(旁)腺切除術具有手術切口小,術后疼痛輕,美容效果好等優(yōu)勢,而且能達到與開放手術相同的治療效果,但內鏡下甲狀(旁)腺切除術的學習曲線相對于傳統(tǒng)開放手術要長。自第一例頸部內鏡手術報道以來,各種各樣的手術入路先后被嘗試,形成了多元化的手術入路方式。每種手術入路有其各自的手術適應癥、禁忌癥、優(yōu)勢和不足,但這些手術入路的共同目的是獲得美觀效果或減少手術創(chuàng)傷。隨著內鏡甲狀(旁)腺切除術的廣泛應用和快速發(fā)展,手術適應癥也隨著技術的發(fā)展不斷擴大,并發(fā)癥發(fā)生率越來越低。選擇合理的手術術式,遵循手術的適應癥和禁忌癥,嚴格把握患者的準入原則,內鏡甲狀(旁)腺切除術可以成為一種安全、有效、可行的治療甲狀腺和甲狀旁腺腫瘤的方法。內鏡甲狀(旁)腺切除術就其美容效果佳、缺陷少等優(yōu)勢已經引起了業(yè)內相關人士的廣泛興趣和關注。
[Abstract]:Thyroid and parathyroid neoplasms are one of the most common tumors in the head and neck. With the development of minimally invasive surgery, minimally invasive surgery and the improvement of patients' aesthetic requirements, the techniques and concepts of minimally invasive surgery have penetrated into the field of thyroid (paraspinal) gland surgery, and endoscopic thyroidectomy has emerged as the times require. Compared with the traditional open thyroid surgery, endoscopic thyroidectomy has the advantages of less incision, less postoperative pain, better cosmetic effect, and can achieve the same therapeutic effect as open surgery. But endoscopic thyroidectomy has a longer learning curve than traditional open surgery. Since the first endoscopic cervical surgery was reported, a variety of surgical approaches have been attempted, forming a variety of surgical approaches. Each approach has its own indications contraindications advantages and disadvantages but the common purpose of these approaches is to achieve aesthetic results or reduce surgical trauma. With the wide application and rapid development of endoscopic thyroidectomy, the operative indications are expanding with the development of technology, and the incidence of complications is becoming lower and lower. It is safe, effective and feasible to use endoscopic thyroidectomy to treat thyroid and parathyroid tumors. Endoscopic thyroidectomy has attracted wide attention for its advantages of good cosmetic effect and less defects.
