【摘要】：目的通過分析2008—2014年北京市以宮頸細(xì)胞學(xué)作為初篩方法行子宮頸癌篩查數(shù)據(jù),了解北京市子宮頸癌篩查效果及存在問題。方法回顧性分析北京市子宮頸癌和乳腺癌篩查個(gè)案信息系統(tǒng)中2008—2009年(第一周期)、2011—2012年(第二周期)、2013—2014年(第三周期)18個(gè)區(qū)縣上報(bào)的子宮頸癌篩查數(shù)據(jù)。結(jié)果第一、二、三周期的篩查覆蓋率分別為19.2%(728 704/3 787 748)、18.8%(495 814/2 630 818)和15.4%(467 863/3 032 690);宮頸細(xì)胞學(xué)陽性(≥ASCUS)檢出率分別為2.3%(16 587/728 704)、2.0%(10 095/495 814)和2.2%(10 073/467 863),其中高級(jí)別宮頸鱗狀上皮內(nèi)病變(HSIL)的檢出率由第一周期的70.4/10萬(513/72 870)增至第三周期的224.2/10萬(1 049/467 863),增加了218.5%;早診率由第一周期的92.0%(552/602)增至第三周期的97.1%(1 061/1 093),增加了5.5%;第一周期宮頸癌檢出率最高(12.2/10萬,89/728 704),第二、三周期為7.5/10萬(37/495 814)和9.4/10萬(44/467 863)。結(jié)論北京市三個(gè)周期宮頸癌前病變的檢出率及早診率逐漸提高,減少了子宮頸癌的發(fā)生,但也存在篩查覆蓋率、宮頸細(xì)胞學(xué)檢出率不高等問題。
