髕骨髕腱結(jié)合部跳躍損傷動物模型建立及運動后冷療對損傷的影響

【摘要】:目的:建立髕骨髕腱結(jié)合部跳躍損傷動物模型和離心運動動物模型,分別用于研究長期和急性跳躍運動對髕骨髕腱結(jié)合部(PPTJ)組織結(jié)構(gòu)、細胞外基質(zhì)和細胞因子的影響,并探討運動后冷療的作用。方法:實驗一:18周齡新西蘭大白兔分為2周組、4周組和對照組,采用兔跳躍裝置進行跳躍訓(xùn)練,每天跳躍150次,每周連續(xù)5天。HE、Safranin o染色顯示組織學(xué)變化,免疫組化染色顯示血管生成、炎性反應(yīng)、損傷修復(fù)等相關(guān)因子的表達。實驗二:分組為運動組、冷療組和對照組,跳躍運動方案同實驗一,觀察運動后冷療對PPTJ組織損傷及肌肉物理特性的影響。實驗三:分組為急性運動組和運動后冷療組和對照組,采用兔離心運動模型進行200次急性落地運動,觀察運動后及運動后冷療2h、24h、48h組織學(xué)變化及促炎性因子和生長因子的表達,并觀察運動后冷療對肌肉物理特性的影響。結(jié)果:實驗一:與對照組相比,4周組局部出現(xiàn)膠原纖維排列紊亂、細胞和粘多糖蛋白分布不均等改變,細胞密度顯著增加(p0.05)、纖維軟骨帶增厚(p0.01),ColⅢ顯著增加(p0.01),ColⅢ/ColⅠ比值顯著增加。IL-6、COX-2、 HIF-1α、VEGF、TGF-β2、BMP、Caspase3陽性染色細胞密度顯著增高(P0.01),SP表達增高(P0.05)。實驗二:運動組和冷療組組織形態(tài)學(xué)指標(biāo)及膠原蛋白表達無明顯差異,但冷療組IL-6和Cox-2表達低于運動組,運動后冷療在第1周可以顯著降低肌肉硬度和肌肉張力。實驗三:急性運動組組織學(xué)指標(biāo)未見顯著改變,IL-6、COX-2在運動后2h達到峰值,Col Ⅰ、ColⅢ/TGF-β2陽性表達在24h時達到峰值,bFGF在48h時達到峰值,均顯著高于對照組(P0.01)。運動后冷療組在運動后2h時COX-2和工L-6表達顯著低于急性運動組,各個時間點上肌肉硬度顯著低于急性運動組(P0.05)。結(jié)論;4周跳躍運動可建立PPTJ跳躍損傷動物模型,缺氧及血管生成、炎性反應(yīng)、Ⅲ膠原表達增高、細胞凋亡增加是PPTJ過度使用性損傷的重要病理特點。運動后冷療對PPTJ損傷發(fā)生未顯示干預(yù)作用。急性或長期運動后冷療可降低炎性反應(yīng),緩解疼痛,并具有一定的降低肌肉硬度作用。
[Abstract]:Objective: to establish the animal models of patellar tendon joint injury and eccentric exercise, and to study the effects of long-term and acute jumping exercise on the tissue structure, extracellular matrix and cytokines of patella patellar tendon. The effect of cold therapy after exercise was discussed. Methods: new Zealand white rabbits at the age of 18 weeks were divided into 2 week group and 4 week group and control group. The jumping training was carried out with rabbit jumping device. The jumping time was 150 times a day. The histopathological changes were observed by Safranin o staining for 5 days a week. Immunohistochemical staining showed the expression of angiogenesis, inflammatory response, injury repair and other related factors. Experiment 2: divided into exercise group, cold therapy group and control group. The jumping exercise scheme was the same as experiment 1. The effects of cold therapy after exercise on PPTJ tissue injury and muscle physical characteristics were observed. Experiment 3: acute exercise group, cold therapy group and control group were divided into acute exercise group, cold therapy group and control group. Two hundred times of acute landing exercise were performed with rabbit centrifugal exercise model. The histological changes and the expression of pro-inflammatory and growth factors were observed at 24 h after exercise and 2 h after cold therapy. The effects of cold therapy after exercise on the physical properties of muscles were observed. Results: experiment 1: compared with the control group, the collagenous fibers were disordered and the distribution of cell and mucopolysaccharide protein was not equal in the 4th week group.
