【摘要】：目的:探討脊柱腱鞘巨細胞瘤和骨巨細胞瘤影像表現(xiàn)異同點,以提高對此兩種腫瘤的認識。方法:對7例病理確診的腱鞘巨細胞瘤和30例骨巨細胞瘤的CT和MRI表現(xiàn)比較分析,總結(jié)其影像學表現(xiàn)的異同點。結(jié)果:7例腱鞘巨細胞瘤中5例位于頸椎以小關節(jié)為中心,2例位于胸椎椎體:3例呈膨脹性骨質(zhì)骨破壞,5例骨破壞區(qū)周圍見硬化邊;30例骨巨細胞瘤中10例位于頸椎、14例位于胸椎、6例位于腰椎,18例呈膨脹性骨質(zhì)破壞;27例腫瘤主體位于椎體,3例位于附件,2例骨破壞周圍可見硬化邊;腫瘤骨破壞部位有統(tǒng)計學差異、骨質(zhì)破壞形式二組無統(tǒng)計學差異;硬化邊形成與否具有統(tǒng)計學差異;4例腱鞘巨細胞瘤在T2WI上呈等低信號、3例呈混雜信號;5例進行增強掃描:1例呈中等強化,4例明顯強化;20例骨巨細胞瘤在T2WI呈等低信號,8例呈混雜信號,2例呈稍高信號;21例進行增強掃描:8例呈明顯均勻強化,12例不均勻增強,1例輕度強化;二組腫瘤的MRI信號、增強特點無統(tǒng)計學差異。結(jié)論:腱鞘巨細胞瘤和骨巨細胞瘤骨破壞形式、MRI信號及增強無統(tǒng)計學差異,而病變部位及硬化形成與否二者具有統(tǒng)計學差異。
[Abstract]:Objective: to investigate the differences and similarities between giant cell tumor of spinal tendon and giant cell tumor of bone. Methods: Ct and MRI findings of 7 cases of pathologically diagnosed giant cell tumor of tendon sheath and 30 cases of giant cell tumor of bone were compared and analyzed. Results of the 7 cases of giant cell tumor of tendon sheath, 5 cases were located in the cervical vertebra with the facet joint as the center. 2 cases were located in the thoracic vertebra vertebral body: 3 cases showed expansive bony bone destruction. 5 cases showed sclerosing margin around the bone destruction area. 10 cases were located in the cervical spine. Cases were located in thoracic vertebrae, 6 cases were located in lumbar vertebrae, 18 cases had expansive bone destruction, 27 cases had tumor body located in vertebral body, 3 cases were located in adnexal bone, 2 cases had sclerosing edge around bone destruction. There was statistical difference in the site of tumor bone destruction, but there was no significant difference in bone destruction between the two groups. There was statistical difference in the formation of sclerosing margin. 4 cases of giant cell tumor of tendon sheath showed iso-low signal intensity on T2WI, 3 cases showed mixed signal and 5 cases underwent enhanced scanning. 1 case showed moderate enhancement and 4 cases showed moderate enhancement. 20 cases of giant cell tumor of bone showed isospermia in T2WI. 8 cases showed mixed signal, 2 cases showed slightly high signal intensity, 21 cases underwent enhanced scan, 8 cases showed obvious homogeneous enhancement, 12 cases showed uneven enhancement, 1 case showed slight enhancement. There was no significant difference in MRI signal enhancement between the two groups. Conclusion: there was no significant difference in MRI signal and enhancement between giant cell tumor of tendon sheath and giant cell tumor of bone.
