【摘要】：目的:分析住院患者替格瑞洛使用情況,了解該藥物的臨床應(yīng)用現(xiàn)狀,為提高替格瑞洛的臨床合理用藥水平提供參考。方法:收集北京大學(xué)第一醫(yī)院2014年1~6月住院期間使用替格瑞洛病歷資料,回顧性分析使用替格瑞洛患者的適應(yīng)癥、用法用量、相互作用及不良反應(yīng)等。結(jié)果:納入分析的患者共36例,男性24例,女性12例;平均年齡(63.77±14.69)歲;體重(70.76±11.17)kg。原發(fā)病均為急性冠脈綜合征、PCI手術(shù)。36例患者中,4例為阿司匹林聯(lián)用替格瑞洛,30例為阿司匹林聯(lián)用氯吡格雷后將氯吡格雷更換為替格瑞洛,1例為阿司匹林聯(lián)用西洛他唑后將西洛他唑更換為替格瑞洛,1例為阿司匹林加用替格瑞洛。31例患者按說(shuō)明書(shū)中所示的用法用量使用,5種患者使用其他不同的用法用量。涉及的可能與替格瑞洛存在相互作用的合并用藥共8種。共6例次出現(xiàn)相關(guān)不良反應(yīng),其中2例(5.56%)發(fā)生出血,4例(11.11%)出現(xiàn)呼吸困難。結(jié)論:我院替格瑞洛多應(yīng)用于對(duì)氯吡格雷低反應(yīng)或其他抗血小板藥物出現(xiàn)不良反應(yīng)的急性冠脈綜合征患者。臨床藥師應(yīng)嚴(yán)格把握該藥用法用量,密切監(jiān)測(cè)患者不良反應(yīng),促進(jìn)替格瑞洛的合理應(yīng)用。
[Abstract]:Objective: To analyze the use of tigloria in hospitalized patients, to understand the clinical application status of the drug, and to provide reference for the improvement of the clinical rational use level of tegareo. Methods: From January to June,2014, the first hospital of Peking University was collected, and the patient's medical records were used to analyze the indications, usage, interaction and adverse reactions of the patients who used the tigloria. Results:36 patients,24 males and 12 females were included in the analysis, mean age (63.77 and 14.69), and body weight (70.76 to 11.17) kg. the original incidence is acute coronary syndrome, In 36 of the 36 patients,4 of the 36 patients were aspirin-combined with tigelorelle,30 were aspirin-combined with chlorofluorocarbon, and 1 case was aspirin-combined with silorello, and the other was replaced with tigrillo, and one case was aspirin, and the other 31 patients were treated with aspirin. The 5 patients used other different usage, as indicated in the instructions for use. There are 8 concomitant medications that may interact with tegrego. There were 6 cases of related adverse reactions, of which 2 (5.56%) had bleeding and 4 (11.11%) had difficulty breathing. Conclusion: The patients with acute coronary syndrome (ACS) with adverse reactions to the low-response or other anti-platelet drugs in the treatment of low-response or other anti-platelet drugs in our hospital. The clinical pharmacist should strictly control the dosage of the medicinal method, closely monitor the adverse reaction of the patient, and promote the rational application of tegareo.
