【摘要】：目的通過我院開展腎功不全患者個體化給藥服務(wù)的實踐,探尋適合基層醫(yī)院推廣個體化給藥服務(wù)的模式,填補(bǔ)個體化給藥服務(wù)的缺失。方法通過臨床調(diào)研了解醫(yī)師對個體化藥學(xué)服務(wù)的需求,設(shè)計具體可行的個體化給藥服務(wù)模式,衍生開展相關(guān)臨床藥學(xué)服務(wù),搭建模塊化的個體化給藥服務(wù)平臺。結(jié)果腎功不全患者個體化給藥設(shè)計的藥學(xué)服務(wù)平臺形成,藥師借此平臺有效參與臨床藥物治療方案的制訂與優(yōu)化,提高了藥物治療水平,保證了特殊患者用藥安全、合理、有效和經(jīng)濟(jì)。結(jié)論腎功能不全患者個體化給藥服務(wù)平臺建立過程簡單,工作模式實用,適宜在西部地區(qū)、少數(shù)民族地區(qū)的基層醫(yī)院推廣,實現(xiàn)個體化給藥的同時,也是深入推進(jìn)臨床藥學(xué)監(jiān)護(hù)的有效載體。
[Abstract]:Objective through the practice of individualized administration of drugs in patients with renal insufficiency in our hospital, to explore the suitable mode of individualized drug delivery in primary hospitals, and to fill in the lack of individualized drug delivery. Methods through the clinical investigation and investigation to understand the doctor's demand for the individualized pharmaceutical service, to design the specific and feasible individualized drug delivery service model, to develop the related clinical pharmaceutical service, and to set up the modularized individualized drug delivery service platform. Results the pharmacological service platform of individualized drug delivery design for patients with renal insufficiency was formed. Pharmacists took part in the formulation and optimization of clinical drug treatment schemes effectively, improved the level of drug treatment, and ensured the safety and reasonableness of drug use in special patients. Efficiency and economy Conclusion the process of establishing individualized drug delivery service platform for patients with renal insufficiency is simple and the working mode is practical. It is suitable to be popularized in grass-roots hospitals in western and ethnic minority areas and to realize individualized drug administration at the same time. It is also an effective carrier for the further promotion of clinical pharmaceutical care.
