【摘要】：目的:了解β-內(nèi)酰胺類抗菌藥物在臨床使用中發(fā)生交叉過(guò)敏反應(yīng)的機(jī)制,為臨床合理用藥提供參考。方法:結(jié)合筆者在美國(guó)伊利諾伊大學(xué)芝加哥分校(UIC)附屬醫(yī)院進(jìn)修期間的學(xué)習(xí)心得,同時(shí)根據(jù)美國(guó)拉什大學(xué)醫(yī)學(xué)中心對(duì)β-內(nèi)酰胺類抗菌藥物過(guò)敏史者的安全用藥管理經(jīng)驗(yàn),總結(jié)β-內(nèi)酰胺類抗菌藥物發(fā)生交叉過(guò)敏反應(yīng)的機(jī)制,并介紹美國(guó)UIC附屬醫(yī)院對(duì)β-內(nèi)酰胺類抗菌藥物過(guò)敏患者的處理流程。結(jié)果:β-內(nèi)酰胺類抗菌藥物發(fā)生交叉過(guò)敏反應(yīng)的主要原因是由于藥物之間存在相同或相似的側(cè)鏈,人體內(nèi)免疫球蛋白E通過(guò)識(shí)別這些側(cè)鏈而產(chǎn)生交叉過(guò)敏反應(yīng)。美國(guó)UIC附屬醫(yī)院對(duì)β-內(nèi)酰胺類抗菌藥物過(guò)敏患者的處理流程,包括對(duì)患者是否有β-內(nèi)酰胺類抗菌藥物使用指征進(jìn)行評(píng)估,根據(jù)評(píng)估結(jié)果進(jìn)行規(guī)范的青霉素皮試,并采用謹(jǐn)慎漸進(jìn)式流程用藥和快速誘導(dǎo)藥物耐受流程進(jìn)行抗感染治療。結(jié)論:美國(guó)UIC附屬醫(yī)院對(duì)β-內(nèi)酰胺類抗菌藥物交叉過(guò)敏反應(yīng)患者的處理方法,可為國(guó)內(nèi)臨床藥師處理可疑β-內(nèi)酰胺類抗菌藥物過(guò)敏史者,尤其是針對(duì)孕婦、兒童等特殊人群提供新的思路。
[Abstract]:Objective: to understand the mechanism of cross-allergic reaction in clinical use of 尾-lactam antibiotics and to provide reference for rational use of 尾-lactam antibiotics. Methods: based on the experience of the author during his study in the (UIC) affiliated Hospital of the University of Illinois, Chicago, USA, and according to the experience of safe drug administration in patients with history of 尾 -lactam antibiotics allergy at Rush University Medical Center. The mechanism of cross-allergic reaction of 尾-lactam antibiotics was summarized, and the treatment process of 尾-lactam antibiotics allergic patients in UIC affiliated hospital of USA was introduced. Results: the main cause of cross allergic reaction of 尾-lactam antibiotics was the existence of the same or similar side chains between the drugs, and immunoglobulin E produced cross allergic reactions by recognizing these side chains in human body. The treatment process of patients with 尾 -lactam antibiotics allergy in the UIC affiliated Hospital of the United States, including the evaluation of whether the patients have indications for the use of 尾 -lactam antibiotics, and the standard penicillin skin test based on the results of the evaluation. The anti-infective therapy was carried out by careful gradual drug use and rapid induction of drug resistance. Conclusion: the treatment of patients with cross allergic reaction of 尾 -lactam antibiotics in UIC affiliated Hospital of the United States can be used by clinical pharmacists to deal with suspected allergic history of 尾 -lactam antibiotics, especially for pregnant women. Special groups such as children provide new ideas.
