【摘要】：唑類抗真菌藥是曲霉病處理中一種最為常用的藥物,而曲霉病主要是由煙曲霉菌(Aspergillus fumigatus)感染引起的。越來越多的報道發(fā)現(xiàn)A.曲霉株(A.fumigatus isolates)具有唑類抗真菌藥耐藥性,這是曲霉病治療中一個巨大的挑戰(zhàn)。A.曲霉唑類抗真菌藥耐藥株(ARAF)在侵襲性曲霉病患者中具有非常高的致死率,這對臨床微生物學者如何及時診斷耐藥性和提出合適的干預策略等方面提出極大的挑戰(zhàn)。ARAF耐藥株既存在cyp51A突變型又存在cyp51A非突變型,而且cyp51A非突變型在唑類抗真菌藥耐藥中占有的比重越來越高,為臨床的診斷和治療提出了更高的挑戰(zhàn)。本綜述總結了目前A.曲霉唑類抗真菌藥耐藥株在全球各國的發(fā)病情況及其耐藥機制研究的最新進展。
[Abstract]:The antifungal agents of azolium are one of the most commonly used drugs in the treatment of aspergillus fumigatus. is the main cause of Aspergillus fumigatus. A growing number of reports found that A. A. fumigatus isolated strains are resistant to antifungal agents, which is a great challenge in the treatment of aspergillosis. Astrazole antifungal resistance strain ARAF has a very high mortality rate in invasive aspergillosis patients. It is a great challenge for clinical microbiologists to diagnose drug resistance in time and put forward appropriate intervention strategies. There are both cyp51A mutation and cyp51A non-mutation in ARAF resistant strains. Moreover, the non-mutagenesis of cyp51A is becoming more and more important in the resistance of antifungal agents, which poses a higher challenge for clinical diagnosis and treatment. This review summarizes the current A. The latest advances in the study of antifungal resistance of Aspergillus in the world.
