【摘要】：目的:對比五水頭孢唑林鈉與頭孢唑林鈉治療細菌感染的成本-效果,為臨床合理用藥提供參考。方法:采用回顧性研究方法,收集2014年11月-2015年12月我院使用五水頭孢唑林鈉或頭孢唑林鈉治療細菌感染的患者207例,其中五水頭孢唑林鈉組109例、頭孢唑林鈉組98例。兩組患者分別給予相應(yīng)的藥物2 g,bid,ivgtt,用藥療程不超過7 d。對比兩組患者的臨床療效、細菌學(xué)療效及安全性,并對兩種治療方案進行藥物經(jīng)濟學(xué)評價。結(jié)果:五水頭孢唑林鈉組和頭孢唑林鈉組患者的臨床有效率分別為89.91%和74.49%,差異有統(tǒng)計學(xué)意義(P0.05);細菌清除率分別為76.32%和72.13%,差異無統(tǒng)計學(xué)意義(P0.05)。兩組患者用藥期間均未見藥物相關(guān)的不良反應(yīng)發(fā)生。五水頭孢唑林鈉組和頭孢唑林鈉組方案的短期(7 d)治療成本分別為4 391.43和3 396.19元,以臨床有效率作為效果指標(biāo)的成本-效果比分別為48.84、45.59,增量成本-效果比為64.55,小于我市人均國內(nèi)生產(chǎn)總值。敏感度分析結(jié)果支持成本-效果分析結(jié)果。結(jié)論:在我市現(xiàn)有經(jīng)濟條件下,使用五水頭孢唑林鈉治療細菌感染較頭孢唑林鈉更具有成本效果優(yōu)勢。
[Abstract]:Objective: to compare the cost-effect of cefazolin sodium pentahydrate and cefazolin sodium in the treatment of bacterial infection. Methods: a retrospective study was conducted in 207 patients with bacterial infection treated with cefazolin pentahydrate or cefazolin sodium in our hospital from November 2014 to December 2015, including 109 cases in cefazolin pentahydrate group and 98 cases in cefazolin sodium group. Patients in the two groups were given 2 g bidobium iv gtt for no more than 7 days. The clinical efficacy, bacteriological efficacy and safety of the two groups were compared. Results: the clinical effective rates of cefazolin sodium pentahydrate group and cefazolin sodium group were 89.91% and 74.49 respectively, the difference was statistically significant (P 0.05), and the bacterial clearance rates were 76.32% and 72.13%, respectively. There were no adverse drug-related reactions in both groups. The treatment cost of cefazolin pentahydrate sodium group and cefazolin sodium group was 4 391.43 yuan and 3 396.19 yuan respectively. The cost to effect ratio of clinical effective rate was 48.84% 45.59, and the incremental cost-effect ratio was 64.55, which was smaller than the per capita gross domestic product (GDP) of our city. Sensitivity analysis results support cost-effect analysis results. Conclusion: cefazolin sodium pentahydrate is more effective than cefazolin sodium in the treatment of bacterial infection.
