【摘要】：目的:探討甲氨蝶呤致藥物過敏反應(yīng)的臨床特點(diǎn),為該藥的安全合理使用提供依據(jù)。方法:報(bào)道我院1例甲氨蝶呤致藥物超敏反應(yīng)綜合征案例,并通過檢索Medline,CNKI,萬方、維普等數(shù)據(jù)庫,收集甲氨蝶呤致過敏反應(yīng)相關(guān)文獻(xiàn)報(bào)道,進(jìn)行統(tǒng)計(jì)分析。結(jié)果:綜合文獻(xiàn)及我院案例,共收集38例甲氨蝶呤致過敏反應(yīng)病例,以18歲以下未成年患者居多,原發(fā)疾病以骨肉瘤和急性淋巴細(xì)胞白血病為主,給藥方式主要為靜脈滴注,臨床表現(xiàn)多為系統(tǒng)性過敏癥狀。除3例患者死亡外,其余均癥狀緩解,并有5例患者經(jīng)脫敏治療后繼續(xù)使用甲氨蝶呤。結(jié)論:甲氨蝶呤相關(guān)過敏反應(yīng)發(fā)生率雖低但嚴(yán)重程度較高,臨床應(yīng)用時(shí)需密切關(guān)注患者的病情變化,及時(shí)采取應(yīng)對措施。對發(fā)生甲氨蝶呤過敏反應(yīng)又須繼續(xù)用藥的患者,可嘗試脫敏治療。
[Abstract]:Objective: to investigate the clinical characteristics of methotrexate induced drug hypersensitivity, and to provide evidence for the safe and rational use of methotrexate. Methods: a case of methotrexate induced drug hypersensitivity syndrome was reported in our hospital. Results: a total of 38 cases of methotrexate induced allergic reaction were collected in our hospital. The primary diseases were osteosarcoma and acute lymphoblastic leukemia. The main way of administration was intravenous drip. The clinical manifestations were mostly systemic hypersensitivity. Five patients continued to use methotrexate after desensitization. Conclusion: the incidence of methotrexate related allergic reaction is low but the severity is high. Timely response. Patients with methotrexate hypersensitivity and need to continue medication, can try desensitization treatment.
