【摘要】：目的:了解江蘇省主要醫(yī)院臨床藥學(xué)工作開(kāi)展現(xiàn)狀,為促進(jìn)臨床藥師制建設(shè)、發(fā)揮臨床藥師在合理用藥和藥學(xué)服務(wù)方面的積極作用提供參考。方法:采用電子郵件方式發(fā)放調(diào)研表,對(duì)江蘇省醫(yī)學(xué)會(huì)臨床藥學(xué)分會(huì)的28位委員(分別來(lái)自28家醫(yī)院)進(jìn)行臨床藥學(xué)工作開(kāi)展現(xiàn)狀調(diào)查,并對(duì)有關(guān)數(shù)據(jù)及結(jié)果進(jìn)行統(tǒng)計(jì)和分析。結(jié)果:共發(fā)放調(diào)研表28份,回收有效調(diào)研表28份,有效回收率為100%。28家受訪醫(yī)院中綜合性醫(yī)院有25家,"三甲"醫(yī)院有26家。共有專兼職臨床藥師312名,平均每家受訪醫(yī)院有7.32名專職臨床藥師和3.82名兼職臨床藥師;專職臨床藥師碩士學(xué)歷、中級(jí)職稱的比例均顯著高于兼職臨床藥師(P0.05)。專職臨床藥師中經(jīng)培訓(xùn)獲得臨床藥師資質(zhì)的共有144人(占70.24%);13家具有國(guó)家衛(wèi)計(jì)委臨床藥師培訓(xùn)基地資格的受訪醫(yī)院平均師資人數(shù)為3.77人,5家具有省級(jí)臨床藥師培訓(xùn)基地資格的受訪醫(yī)院平均師資人數(shù)為5.80人,1家具有國(guó)家衛(wèi)計(jì)委臨床藥師師資培訓(xùn)基地資格的受訪醫(yī)院師資人數(shù)為5人。受訪醫(yī)院開(kāi)展臨床藥學(xué)相關(guān)工作以參與日常醫(yī)師查房、參與醫(yī)囑和處方點(diǎn)評(píng)、對(duì)抗菌藥物進(jìn)行專項(xiàng)點(diǎn)評(píng)、書(shū)寫藥歷、藥學(xué)信息服務(wù)、門診藥物咨詢和藥品不良反應(yīng)監(jiān)測(cè)的比例最高,均為100%,而以專家咨詢門診比例最低(占10.71%),其他比例較低的項(xiàng)目有開(kāi)設(shè)手麻科藥房(占28.57%)、社區(qū)藥學(xué)服務(wù)(占57.14%)、藥物基因組學(xué)檢測(cè)(占46.43%)和參與管理藥物臨床試驗(yàn)(占53.57%)等;培訓(xùn)基地醫(yī)院臨床藥學(xué)各項(xiàng)工作開(kāi)展比例總體高于非培訓(xùn)基地醫(yī)院,受訪省級(jí)臨床藥師培訓(xùn)基地醫(yī)院抗腫瘤藥物、激素類藥物和血液制品專項(xiàng)點(diǎn)評(píng)及專家咨詢門診、開(kāi)設(shè)手麻科藥房、社區(qū)藥學(xué)服務(wù)、藥物基因組學(xué)檢測(cè)、參與管理藥物臨床試驗(yàn)等項(xiàng)開(kāi)展比例與受訪醫(yī)院比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);平均每家省級(jí)臨床藥師培訓(xùn)基地醫(yī)院與平均每家受訪醫(yī)院比較,在�？茣�(huì)診次數(shù)、病案分析例次、住院病患注射劑醫(yī)囑審核份數(shù)、靜脈配置醫(yī)囑審核條數(shù)、門診處方審核張數(shù)、治療藥物監(jiān)測(cè)品種和例數(shù)以及藥物基因組學(xué)檢測(cè)例數(shù)上,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:江蘇省"三甲"綜合性醫(yī)院臨床藥師隊(duì)伍建設(shè)有待加強(qiáng),兼職臨床藥師還需加速轉(zhuǎn)型,同時(shí)臨床藥學(xué)相關(guān)工作開(kāi)展水平尚待進(jìn)一步提高。
[Abstract]:Objective: to understand the main hospital of Jiangsu Province, the work of clinical pharmacy status, in order to promote clinical pharmacist system construction, play a positive role of clinical pharmacists in rational drug use and pharmaceutical services to provide the reference. Methods: using e-mail issued investigation table of Jiangsu Province Medical Association Clinical Pharmacy branch 28 members (from 28 hospitals) this paper surveys the current situation of clinical pharmacy work, and analyzed the relevant data and results. Results: the survey issued a total of 28 copies, 28 copies of valid investigation table, the effective recovery rate was 100%.28 in comprehensive hospitals surveyed 25 hospitals, hospital of 26. A total of part-time clinical pharmacists 312, average hospitals surveyed 7.32 full-time and 3.82 part-time clinical pharmacists of clinical pharmacist; clinical pharmacists professional master degree, intermediate rates were significantly higher than those in part-time clinical pharmacists (P0.05 ). The full-time clinical pharmacists in clinical pharmacist qualification obtained by training a total of 144 people (70.24%); 13 furniture with the State Planning Commission of clinical pharmacist training base qualifications of the hospitals surveyed teachers average number of 3.77 people, 5 level with the clinical pharmacist training base qualifications of the hospitals surveyed average teachers for the number 5.80, 1 furniture with the State Planning Commission clinical pharmacists training base qualifications of the hospitals surveyed teachers for the number 5 hospitals surveyed. Clinical pharmacy related work to participate in the daily round, in order and prescription review, special comments on the writing of antimicrobial drugs, drug history, pharmaceutical information services, outpatient consultation and adverse drug drug monitoring response to the highest proportion was 100%, while the lowest proportion of outpatients with expert consultation (10.71%), Department of pharmacy has set up a lower proportion of other military projects (28.57%), community pharmacy services (57.14%), drug From genomics testing (46.43%) and participate in the management of drug clinical trials (53.57%); training base hospital clinical pharmacy work carried out higher than the proportion of the overall training base hospital, interviewed the provincial clinical pharmacist training base hospital antitumor drugs, hormone drugs and blood products special comments and expert consultation, open hands, community pharmacy services, pharmacy, pharmacogenomic testing, to participate in the management of drug clinical trial, conducted and the proportion of hospitals surveyed, there were statistically significant differences (P0.05); the average level of clinical pharmacist training base hospital and average hospitals surveyed, in the number of specialist consultation, case analysis cases, inpatient injection the number of copies of the medical examination, the medical examination number of intravenous transfusion, outpatient prescription audit a number of therapeutic drug monitoring varieties and the number of cases as well as drug genomics testing cases The difference was statistically significant (P0.05). Conclusion: the construction of clinical pharmacist team in Jiangsu's "three a" comprehensive hospital needs to be strengthened. Part time clinical pharmacists need to accelerate transformation. At the same time, the level of clinical pharmacy work needs further improvement.
