【摘要】：目的:探討血清ADAMTS-7在冠心病(CHD)患者中的表達水平,分析其與Gensini評分的相關性。方法:連續(xù)選取2016年4月至2017年2月在山西省人民醫(yī)院心內科一病區(qū)住院的胸痛待查者389例,根據“納入和排除標準”,入組患者共151例,分為冠脈正常且無ST-T改變者31例,CHD組120例。所有入選病例統(tǒng)一檢測血脂、血糖、肝腎功、超敏C反應蛋白等生化指標,ELISA法測定血清ADAMTS-7的水平。按冠脈造影(CAG)結果計算Gensini積分,以Gensini評分結果將CHD組分為Ⅰ組(4~8分)37例,Ⅱ組(9~12分)23例,Ⅲ組(13~33分)31例,Ⅳ組(34分及以上)29例;以及Gensini積分為0的非CHD組共5組。應用兩獨立樣本t檢驗評價CHD患者血清中ADAMTS-7水平,五組間ADAMTS-7水平比較應用方差分析(ANOVA),分析ADAMTS-7與Gensini積分的相關性應用Spearman偏相關。結果:1.CHD組的血清ADAMTS-7水平高于非CHD組,即[(71.44±27.33)ng/ml vs(36.17±14.02)ng/ml,P0.001]。2.CHD患者中Gensini積分Ⅳ組(34分及以上),Ⅲ組(13~33分),Ⅱ組(9~12分),Ⅰ組(4~8分)的血清ADAMTS-7水平均高于非CHD組的水平[(97.06±27.83)ng/ml,(75.38±25.40)ng/ml,(58.64±14.15)ng/ml,(56.01±17.76)ng/ml vs(36.17±14.02)ng/ml,P0.001]。CHD組內比較時,Ⅱ組的血清ADAMTS-7的水平略高于Ⅰ組,但無統(tǒng)計學差異(P0.05);剩余的Ⅳ組、Ⅲ組的血清ADAMTS-7水平均明顯高于Ⅱ組、Ⅰ組,差異有統(tǒng)計學意義(P0.05);Ⅳ組與Ⅲ組之間的比較,差異也具有統(tǒng)計學意義(P0.05)。3.在調整了其他混雜因素后,ADAMTS-7水平與Gensini評分之間仍存在顯著相關性(r=0.479,P0.001)。結論:CHD患者血清ADAMTS-7水平明顯升高,隨著Gensini積分的增高,ADAMTS-7的水平逐漸增高,ADAMTS-7水平與Gensini評分呈正相關,提示血清ADAMTS-7水平可作為評價冠脈病變程度的一個重要生化指標。
[Abstract]:Aim: to investigate the expression of serum ADAMTS-7 in patients with coronary heart disease (CHD) and to analyze the correlation between ADAMTS-7 and Gensini score. Methods: a total of 389 patients with chest pain were selected from April 2016 to February 2017 in the Department of Cardiology, Shanxi Provincial people's Hospital. According to the criteria of inclusion and exclusion, 151 patients were enrolled. The patients were divided into 31 patients with normal coronary artery and no ST-T change, 120 patients with CHD. The serum levels of ADAMTS-7 were determined by Elisa in all the patients, including blood lipids, blood glucose, liver and kidney function, and hypersensitive C-reactive protein.
