桃紅四物湯合五味消毒飲加減治療全膝關(guān)節(jié)置換術(shù)后炎癥反應(yīng)療效的臨床研究

【摘要】：目的：研究桃紅四物湯合五味消毒飲加減治療全膝關(guān)節(jié)置換術(shù)后炎癥反應(yīng)的臨床療效,有利于為臨床提供一種療效更佳的綜合療法。方法：選擇符合標(biāo)準(zhǔn)的行初次單側(cè)全膝關(guān)節(jié)置換術(shù)患者70例,用統(tǒng)計軟件隨機分為2組：實驗組(35例)和對照組(35例)。對照組術(shù)后予常規(guī)治療,實驗組在對照組常規(guī)治療基礎(chǔ)上加服桃紅四物湯合五味消毒飲加減(每天1劑,連服14天)。分別于術(shù)前1天、術(shù)后3、7、14天對比兩組患者血清TNF-α、IL-6、CRP、膝關(guān)節(jié)腫脹程度、視覺模擬疼痛評分及關(guān)節(jié)活動度。結(jié)果：1.血清TNF-α:組間比較：術(shù)前兩組差異無統(tǒng)計學(xué)意義(P0.05)；術(shù)后3、7、14天實驗組低于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。組內(nèi)比較：兩組均有術(shù)后7天低于術(shù)后3天、術(shù)后14天低于術(shù)后7天,差異均有統(tǒng)計學(xué)意義(P0.05)。2.血清IL-6：組間比較：術(shù)前兩組差異無統(tǒng)計學(xué)意義(P0.05)；術(shù)后3、7、14天實驗組低于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。組內(nèi)比較：兩組均有術(shù)后7天低于術(shù)后3天、術(shù)后14天低于術(shù)后7天,差異均有統(tǒng)計學(xué)意義(P0.05)。3.血清CRP：組間比較：術(shù)前兩組差異無統(tǒng)計學(xué)意義(P0.05)；術(shù)后3、7、14天實驗組低于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。組內(nèi)比較：兩組均有術(shù)后7天低于術(shù)后3天、術(shù)后14天低于術(shù)后7天,差異均有統(tǒng)計學(xué)意義(P0.05)。4.膝關(guān)節(jié)腫脹程度：組間比較：術(shù)前兩組差異無統(tǒng)計學(xué)意義(P0.05)；術(shù)后3、7、14天實驗組低于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。組內(nèi)比較：兩組均有術(shù)后7天低于術(shù)后3天、術(shù)后14天低于術(shù)后7天,差異均有統(tǒng)計學(xué)意義(P0.05)。5.膝關(guān)節(jié)視覺模擬疼痛評分：組間比較：術(shù)前兩組差異無統(tǒng)計學(xué)意義(P0.05)；術(shù)后3、7天實驗組低于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。組內(nèi)比較：兩組均有術(shù)后7天低于術(shù)后3天、術(shù)后14天低于術(shù)后7天,差異均有統(tǒng)計學(xué)意義(P0.05)。6.膝關(guān)節(jié)活動度：組間比較：術(shù)前兩組差異無統(tǒng)計學(xué)意義(P0.05)；術(shù)后3、7天實驗組優(yōu)于對照組,差異均有統(tǒng)計學(xué)意義(P0.05)。組內(nèi)比較：兩組均有術(shù)后7天優(yōu)于術(shù)后3天、術(shù)后14天優(yōu)于術(shù)后7天,差異均有統(tǒng)計學(xué)意義(P0.05)。結(jié)論：桃紅四物湯合五味消毒飲加減可有效治療全膝關(guān)節(jié)置換術(shù)后炎癥反應(yīng),有助于機體康復(fù)。
[Abstract]:Objective: to study the clinical effect of Taohong Siwu decoction combined with Wuwei disinfecting decoction in the treatment of inflammatory reaction after total knee arthroplasty, which is beneficial to provide a better comprehensive therapy for clinic. Methods: 70 patients undergoing initial unilateral total knee arthroplasty were randomly divided into two groups: experimental group (n = 35) and control group (n = 35). The control group was treated with routine treatment after operation, and the experimental group was treated with Taohong Siwu decoction combined with Wuwei Disinfection decoction on the basis of routine treatment in the control group (1 dose per day for 14 days). The swelling degree of serum TNF- 偽 and IL-6,CRP, knee joint, visual simulated pain score and joint range of motion were compared between the two groups 1 day before operation, 3 days after operation, 7 days after operation and 14 days after operation. Results: 1. Serum TNF- 偽: there was no significant difference between the two groups before operation (P 0.05), and the experimental group was lower than the control group at 3, 7 and 14 days after operation, and the difference was statistically significant (P 0.05). Intra-group comparison: 7 days after operation was lower than 3 days after operation, 14 days after operation was lower than 7 days after operation, the difference was statistically significant (P 0.05). Intra-group comparison: 7 days after operation was lower than 3 days after operation, 14 days after operation was lower than 7 days after operation, the difference was statistically significant (P 0.05). Knee joint range of motion: there was no significant difference between the two groups before operation (P 0.05), and the experimental group was superior to the control group on the 3rd and 7th day after operation, and the difference was statistically significant (P 0.05). Intra-group comparison: 7 days after operation was better than 3 days after operation, 14 days after operation was better than 7 days after operation, the difference was statistically significant (P 0.05). Conclusion: Taohong Siwu decoction combined with Wuwei Disinfection decoction can effectively treat the inflammatory reaction after total knee arthroplasty and contribute to the rehabilitation of the body.
