【摘要】：目的:探討Duchenne型肌營(yíng)養(yǎng)不良合并脊柱側(cè)凸的相關(guān)危險(xiǎn)因素,為Duchenne型肌營(yíng)養(yǎng)不良合并脊柱側(cè)凸的病情判斷提供依據(jù)。方法:選取Duchenne型肌營(yíng)養(yǎng)不良患者200例,其中并發(fā)脊柱側(cè)凸79例為病例組,而121例未并發(fā)脊柱側(cè)凸為對(duì)照組,采用單因素χ2檢驗(yàn)和多因素Logistic回歸分析Duchenne型肌營(yíng)養(yǎng)不良合并脊柱側(cè)凸的相關(guān)危險(xiǎn)因素。結(jié)果:多因素分析顯示下肢肌力不對(duì)稱(OR=3.545,95CI:1.242~10.115),獨(dú)立行走困難(OR=16.688,95CI:5.036~55.297),未堅(jiān)持康復(fù)鍛煉(OR=0.068,95CI:0.015~0.305),跟腱攣縮形成馬蹄內(nèi)翻足(OR=1.597,95CI:0.462~5.524),未使用激素治療(OR=1.464,95CI:1.437~5.439)。結(jié)論:下肢肌力不對(duì)稱,獨(dú)立行走困難,未堅(jiān)持康復(fù)鍛煉,跟腱攣縮形成馬蹄內(nèi)翻足,未使用激素治療等是Duchenne型肌營(yíng)養(yǎng)不良合并脊柱側(cè)凸的獨(dú)立危險(xiǎn)因素,臨床上對(duì)有上述危險(xiǎn)因素的Duchenne型肌營(yíng)養(yǎng)不良患者應(yīng)注意預(yù)防脊柱側(cè)凸,避免發(fā)生脊柱側(cè)凸造成不良后果。
[Abstract]:Objective: to investigate the risk factors of Duchenne muscular dystrophy with scoliosis and to provide evidence for the diagnosis of Duchenne muscular dystrophy with scoliosis. Methods: 200 cases of Duchenne muscular dystrophy were selected, among them 79 cases were complicated with scoliosis and 121 cases were not complicated with scoliosis as control group. Univariate 蠂 2 test and multivariate logistic regression analysis were used to analyze the risk factors of Duchenne muscular dystrophy with scoliosis. Results: multivariate analysis showed that the lower extremity muscle strength was asymmetrical (ORN 3.54595 CI: 1.242J 10.115), the difficulty of independent walking was difficult (ORL 16.68895 CI: 5.03655.297), the rehabilitation exercise was not persisted (ORN 0.068 95CI0.0150.305), the contracture of Achilles tendon resulted in equinovarus varus (OR1.59795 CI: 0.4622.524), and no hormone therapy was used (ORL 1.46495 CI: 1.4375.439). Conclusion: the independent risk factors of Duchenne muscular dystrophy combined with scoliosis are asymmetry of lower limb muscle strength, difficulty of independent walking, failure to insist on rehabilitation exercise, formation of equinovarus foot caused by Achilles tendon contracture and no hormone therapy. Prevention of scoliosis should be paid attention to in patients with Duchenne muscular dystrophy with the above mentioned risk factors, and the adverse consequences of scoliosis should be avoided.
