全髖關(guān)節(jié)置換術(shù)后股骨偏心距與聚乙烯磨損的相關(guān)性研究

【摘要】:[目的]探討人工全髖關(guān)節(jié)置換術(shù)后股骨偏心距與聚乙烯磨損的相關(guān)性。[方法]用計(jì)算機(jī)輔助測(cè)量方法測(cè)量髖關(guān)節(jié)置換聚乙烯內(nèi)襯的線性磨損。以股骨頭中心參照于髖臼杯開口面軸線的位移,計(jì)算線性磨損的大小與方向。對(duì)2008年11月~2015年10月本院骨科78例使用非骨水泥型金屬對(duì)聚乙烯假體患者的X線片進(jìn)行股骨偏心距和聚乙烯磨損的測(cè)量,平均隨訪時(shí)間4.08年。[結(jié)果]平均線性磨損率(0.18±0.04)mm/年,平均線性磨損方向與髖臼杯軸線的夾角(33.0±18.2)°。術(shù)中股骨偏心距得到重建組的線性磨損率小于未得到重建組(P0.001)。股骨偏心距得到重建組與未得到重建組的磨損方向差異無統(tǒng)計(jì)學(xué)意義(P=0.210)。對(duì)股骨雙側(cè)偏心距差值與年磨損率進(jìn)行相關(guān)分析,兩者成正相關(guān)性(r=0.669,P0.001)。[結(jié)論]人工全髖關(guān)節(jié)置換術(shù)中重建股骨偏心距可減慢聚乙烯內(nèi)襯磨損速度,延長(zhǎng)髖關(guān)節(jié)假體的壽命,改善遠(yuǎn)期效果。
[Abstract]:Objective: to investigate the correlation between femoral eccentricity and polyethylene wear after total hip arthroplasty. [methods] Linear wear of PE liner was measured by computer aided measurement. The size and direction of linear wear were calculated by reference to the displacement of the opening axis of the acetabular cup at the center of the femoral head. From November 2008 to October 2015, 78 cases of orthopedic patients in our hospital using non-bone cement type metal to measure the femoral eccentricity and polyethylene wear of polyethylene prosthesis were studied. The average follow-up time was 4.08 years. [results] the average linear wear rate was 0. 18 鹵0.04)mm/ years, and the angle between the average linear wear direction and the acetabular cup axis was 33. 0 鹵18. 2 擄. The linear wear rate of the reconstruction group was lower than that of the non-reconstructed group (P 0.001). There was no significant difference in wear direction between the reconstruction group and the unreconstructed group. The correlation analysis between the difference of bilateral eccentric distance of femur and the annual wear rate showed that there was a positive correlation between the difference of eccentric distance of femur and the annual wear rate. [conclusion] the reconstruction of femoral eccentricity in total hip arthroplasty can slow down the wear rate of polyethylene lining, prolong the life of hip prosthesis and improve the long-term effect.
