【摘要】：[目的]分析骨科手術(shù)切口多重耐藥菌感染的相關(guān)危險(xiǎn)因素,為臨床采取有效措施提供參考。[方法]收集2011年6月~2015年12月骨科發(fā)生切口感染372例患者的臨床資料,對(duì)其多重耐藥菌感染的相關(guān)因素進(jìn)行單因素與多因素統(tǒng)計(jì)分析。[結(jié)果]骨科手術(shù)4 651例,發(fā)生切口感染372例,感染發(fā)病率8.00%,住院時(shí)間≥10 d、營(yíng)養(yǎng)不良、手術(shù)類(lèi)型(急診手術(shù))、手術(shù)種類(lèi)(有內(nèi)置物)、手術(shù)部位(腰腹以下及下肢)、ASA分級(jí)(Ⅲ~Ⅳ級(jí))、手術(shù)時(shí)間2 h、切口長(zhǎng)度15 cm、出血量≥1 500 ml、損傷性治療、臥床時(shí)間≥5 d、抗菌藥物使用天數(shù)≥10 d、種類(lèi)≥3種、聯(lián)用3種以上抗菌藥物、血紅蛋白90 g/L均是骨科手術(shù)切口多重耐藥菌感染的危險(xiǎn)因素(P0.05),其中營(yíng)養(yǎng)不良(OR2.231,95%CI1.100-4.524)、手術(shù)種類(lèi)(有內(nèi)置物)(OR3.773,95%CI1.718-8.288)、抗菌藥物使用天數(shù)≥10 d(OR2.434,95%CI1.218-4.865)、聯(lián)用3種以上抗菌藥物(OR8.905,95%CI1.388-57.136)是骨科切口感染的獨(dú)立危險(xiǎn)因素。[結(jié)論]骨科切口感染與多種危險(xiǎn)因素有關(guān),其中營(yíng)養(yǎng)不良、手術(shù)種類(lèi)(有內(nèi)置物)、抗菌藥物使用天數(shù)≥10 d和聯(lián)用3種以上抗菌藥物是骨科切口感染的獨(dú)立危險(xiǎn)因素。
[Abstract]:Objective: to analyze the risk factors of multidrug resistant bacteria infection in orthopedic surgical incision. [methods] the clinical data of 372 patients with incision infection in orthopedic department from June 2011 to December 2015 were collected, and the related factors of multidrug resistant bacteria infection were analyzed by univariate and multivariate statistical analysis.
