【摘要】：目的:比較支撐鋼板與鎖定鋼板治療復雜脛骨平臺骨折的療效。方法:選取在我院治療的復雜脛骨平臺骨折患者45例,時間為2011年9月~2014年2月,其中25例行雙側入路鎖定鋼板治療,為A組,雙側支撐鋼板內固定治療20例,為B組。對比兩組的術中出血量、手術時間、完全負重時間有無區(qū)別,最后一次隨訪應用Rasmussen評分標準對膝關節(jié)功能情況進行評估來比較兩組的膝關節(jié)優(yōu)良率。結果:術后定期對患者進行隨訪,1例失訪,兩組患者骨折均正常愈合,沒有發(fā)生骨折延期愈合、不愈合情況;A組患者的手術時間(138.8±2.9)min、術中出血量(313.9±3.9)ml,B組患者的手術時間(136.7±3.3)min、術中出血量(317.1±3.8)ml,差異無有統(tǒng)計學意義(P0.05);但A組患者的完全負重時間(3.8±0.2)月明顯短于B組的完全負重時間(4.7±0.3)月,A組膝關節(jié)功能優(yōu)良率88%高于B組的70%,差異有統(tǒng)計學意義(P0.05)。結論:鎖定鋼板組更符合生物力學原則,完全負重時間明顯早于支撐鋼板組,有利于早期膝關節(jié)功能鍛煉,膝關節(jié)優(yōu)良率也高于支撐鋼板組。相對于支撐鋼板,鎖定鋼板在早期下地鍛煉、改善膝關節(jié)功能方面有明顯優(yōu)勢。
[Abstract]:Objective: to compare the curative effect of supporting plate and locking plate in the treatment of complex tibial plateau fracture. Methods: Forty-five patients with complex tibial plateau fractures treated in our hospital from September 2011 to February 2014 were selected. 25 cases were treated with locking plate with bilateral approach, group A and 20 cases with bilateral braced plate internal fixation. The amount of blood loss, time of operation and time of full weight bearing were compared between the two groups. The excellent and good rate of knee joint in the two groups was evaluated by using Rasmussen score in the last follow-up. Results: one patient was followed up regularly after operation. The fracture healed normally in both groups, and there was no delayed union. The operative time was 138.8 鹵2.9 min in group A, 136.7 鹵3.3 min in group B and 317.1 鹵3.8 ml in group B, but the time of complete weight loading in group A was significantly shorter than that in group B (3.8 鹵0.2) months. The excellent and good rate of knee joint function in group A was 88% higher than that in group B (4.7 鹵0.3) months, and the difference was statistically significant (P 0.05). Conclusion: locking plate group is more in line with biomechanical principle, the time of full weight loading is earlier than that of support plate group, which is beneficial to early knee joint function exercise, and the excellent and good rate of knee joint is higher than that of support plate group. Compared with supporting plate, locking plate has obvious advantages in early exercise and improvement of knee joint function.
