【摘要】：目的:為臨床術(shù)后合理選擇鎮(zhèn)痛藥物及優(yōu)化鎮(zhèn)痛方案提供參考。方法:從臨床藥師的角度,對(duì)2016年美國(guó)《術(shù)后疼痛管理指南》(簡(jiǎn)稱(chēng)《指南》)第6～22章涉及術(shù)后疼痛治療藥物的內(nèi)容進(jìn)行解讀。結(jié)果:《指南》涉及術(shù)后疼痛治療藥物的內(nèi)容包括圍術(shù)期治療藥物與給藥途徑的選擇、多模式鎮(zhèn)痛給藥方案、個(gè)體化藥物治療方案,以及患者的用藥監(jiān)護(hù)、用藥教育、術(shù)后疼痛管理方案等�！吨改稀凡煌扑]在術(shù)前給予患者阿片類(lèi)藥物或非選擇性非甾體抗炎藥,因?yàn)椴荒塬@益;對(duì)于術(shù)后可口服給藥的患者,若給予阿片類(lèi)藥物應(yīng)優(yōu)先選擇口服制劑,避免采用肌內(nèi)注射方式給藥�！吨改稀窂�(qiáng)推薦采用多模式鎮(zhèn)痛治療方案與非藥物性干預(yù)相結(jié)合的手段治療患者術(shù)后疼痛,并根據(jù)患者的具體病情選擇個(gè)體化治療方案;臨床醫(yī)師應(yīng)重點(diǎn)監(jiān)護(hù)術(shù)后患者的鎮(zhèn)靜程度、呼吸狀態(tài)及不良反應(yīng)發(fā)生情況,并應(yīng)向患者及其家屬講解術(shù)后疼痛管理流程及出院后鎮(zhèn)痛藥物的減量方法。結(jié)論:《指南》對(duì)圍術(shù)期患者鎮(zhèn)痛給藥方案的規(guī)定,可為臨床醫(yī)師選擇合適的個(gè)體化藥物鎮(zhèn)痛方案提供參考。
[Abstract]:Objective: to provide a reference for the rational selection of analgesic drugs and the optimization of analgesic scheme after operation. Methods: from the point of view of clinical pharmacists, In 2016, the American guidelines for postoperative pain Management (hereinafter referred to as "the guidelines") Chapter 622 related to postoperative pain treatment drugs were interpreted. Results: the contents of the guidelines related to postoperative pain treatment drugs, including perioperative treatment drugs. And the choice of drug delivery route, Multimode analgesic regimen, individualized drug therapy, patient monitoring, drug education, postoperative pain management program, etc. The guidelines do not recommend that patients be given opioids or non-selective non-steroidal anti-inflammatory drugs before operation. Because there is no benefit; for patients who can be administered orally after surgery, opioids should be given priority to oral preparations, Avoid intramuscular injection. The guidelines strongly recommend the use of multi-mode analgesic therapy combined with non-drug intervention in the treatment of postoperative pain, and according to the specific condition of patients to choose individualized treatment; Clinicians should focus on monitoring the sedation, respiratory status and adverse reactions of the patients after surgery. The procedure of postoperative pain management and the method of reducing analgesic drugs after discharge should be explained to the patients and their families. It can provide reference for clinicians to select appropriate individual analgesic regimen.
