【摘要】：目的研究多個小切口聯(lián)合負壓治療對毒蛇咬傷后腫脹消退和炎癥反應的影響。方法選擇2015年4月至2016年7月我科收治的毒蛇咬傷患者29例,其中男性18例,女性11例,年齡18~70歲,用隨機數(shù)字法將研究對象分為對照組和觀察組(注冊號:Chi CTR-TRC-14004189)。對照組采用傳統(tǒng)綜合治療,觀察組行患肢近端皮膚多個小切口手術后予以負壓吸引,同時采用傳統(tǒng)綜合治療。分別在治療前(0 h),治療后6、12、24、48、72 h 6個時間點,測量并計算患肢近端、中部、遠端肢體周徑與健側對應部位肢體周徑差值,同時測血清IL-6和TNF-α濃度,記錄平均住院時間。結果不同時間下測量的觀察組與對照組肢體周徑差值差異有統(tǒng)計學意義,觀察組小于對照組(P0.05);在不同測量時間下對照組與觀察組IL-6與TNF-α濃度差異有統(tǒng)計學意義(P0.05),觀察組低于對照組;兩組平均住院時間差異有統(tǒng)計學意義(P=0.001),觀察組少于對照組[觀察組(5.43±1.07)d,對照組(7.67±1.95)d]。結論毒蛇咬傷后在傳統(tǒng)綜合治療基礎上采用小切口負壓治療可有效控制腫脹,加速腫脹消退,減輕全身炎癥反應,縮短住院時間。
[Abstract]:Objective to study the effect of multiple small incision combined with negative pressure therapy on swelling and inflammation after venomous snake bite. Methods 29 patients with venomous snake bite were selected from April 2015 to July 2016, including 18 males and 11 females. Aged 1870 years, the subjects were divided into control group and observation group (registration number: 1: Chi CTR-TRC-14004 1899). The control group was treated with traditional comprehensive therapy, and the observation group received several small incision operations on the proximal skin of the affected limb to attract negative pressure. At the same time, the traditional comprehensive therapy was used. The difference of circumference diameter between the proximal and middle extremities and the corresponding parts of the contralateral limbs were measured and calculated at 6 time points, 6 hours before treatment and 6 hours after treatment. The serum concentrations of IL-6 and TNF- 偽 were measured at the same time, and the difference between the peripheral diameter of the distal extremity and the corresponding part of the contralateral extremity was measured, and the serum IL-6 and TNF- 偽 concentrations were measured. Results the difference of circumference between observation group and control group was statistically significant. The concentration of IL-6 and TNF- 偽 in the observation group was significantly lower than that in the control group (P 0.05), and was lower in the observation group than in the control group. The difference of average hospitalization time between the two groups was statistically significant, and the observation group was less than the control group [the observation group was 5.43 鹵1.07 days, the control group was 7.67 鹵1.95 days]. Conclusion the small incision negative pressure therapy after snake bite can effectively control the swelling and accelerate the swelling regression. Reduce systemic inflammatory reaction and shorten hospital stay.
