帕金森病患者靜息態(tài)腦功能局部一致性研究

【摘要】:目的探討帕金森病(PD)患者靜息態(tài)腦功能局部一致性(ReHo)的變化。方法采集22例原發(fā)性PD患者(PD組)和22名健康對照者(正常對照組)的靜息態(tài)功能磁共振(fMRI)數(shù)據(jù),并進行比較。分析PD患者"開"期和"關"期局部一致性(ReHo)值差異有統(tǒng)計學意義的腦區(qū)的Re Ho值與PD患者左手對指改善率的相關性。結(jié)果與正常對照組比較,PD組"關"期ReHo值的減少主要集中在左側(cè)殼核、雙側(cè)小腦半球,而增加的腦區(qū)集中在右側(cè)丘腦、左側(cè)額中回、左側(cè)運動前區(qū)、右側(cè)頂下小葉、右側(cè)中央后回、雙側(cè)輔助運動區(qū)、楔前葉。PD組患者"開"期雙側(cè)小腦、顳下回、右側(cè)殼核、右側(cè)楔前葉、右側(cè)丘腦、雙側(cè)輔助運動區(qū)ReHo值較"關"期顯著增加;而右側(cè)小腦后葉、右側(cè)顳中回、右側(cè)顳上回、左側(cè)額中回、左側(cè)額上回、右側(cè)后扣帶回較"關"期顯著。PD患者左手對指改善率為1.59%~126.67%,平均(54.15±38.02)%。Pearson相關性分析結(jié)果顯示,PD患者右側(cè)丘腦ReHo值與左手對指改善率呈正相關(r=0.637,P0.01)。結(jié)論 PD患者靜息態(tài)腦功能存在廣泛異常,左旋多巴對于PD患者的大腦功能環(huán)路具有修飾作用。丘腦作為運動環(huán)路的一個重要節(jié)點,在PD患者中其神經(jīng)元代謝、功能等也發(fā)生了改變。
[Abstract]:Objective to investigate the changes of resting brain function in patients with Parkinson's disease (PD). Methods the resting functional magnetic resonance imaging (fMRI) data of 22 patients with primary PD and 22 healthy controls (normal control group) were collected and compared. To analyze the correlation between the value of re Ho in the brain region of PD patients and the improvement rate of left hand contralateral finger in PD patients. Results in PD group, the decrease of ReHo was mainly in the left putamen nucleus and bilateral cerebellar hemisphere, while the increased brain area was concentrated in the right thalamus, left middle frontal gyrus, left premotor area and right inferior parietal lobule. The ReHo values of bilateral cerebellum, inferior temporal gyrus, right putamen nucleus, right anterior cuneate lobe, right thalamus and bilateral auxiliary motor area in patients with right posterior central gyrus, bilateral auxiliary motor area and bilateral auxiliary motor area were significantly higher than those in "Guan" phase. The right posterior lobe of the cerebellum, the right middle temporal gyrus, the right superior temporal gyrus, the left middle frontal gyrus, the left superior frontal gyrus, The improvement rate of right posterior cingulate gyrus was significantly higher than that of "Guan" stage. The improvement rate of left hand finger in PD patients was 1.596.67. The results of correlation analysis showed that there was a positive correlation between the ReHo value of right thalamus of PD patients and the improvement rate of left contralateral finger. Conclusion there are extensive abnormalities in resting brain function in PD patients. Levodopa can modify the cerebral functional loop in PD patients. As an important node of motor loop, the thalamus has changed its neuronal metabolism and function in PD patients.
