酸冰刺激療法與神經(jīng)肌肉電刺激療法聯(lián)合應(yīng)用對腦卒中后吞咽障礙的改善效果觀察

【摘要】:目的觀察酸冰刺激療法與神經(jīng)肌肉電刺激療法聯(lián)合應(yīng)用對腦卒中后吞咽障礙的改善效果。方法116例腦卒中后吞咽功能障礙患者隨機分為觀察組、對照組各58例,兩組均采取常規(guī)治療及神經(jīng)肌肉電刺激療法治療,觀察組在此基礎(chǔ)上加用酸冰刺激療法治療,每個療程20 d,持續(xù)治療2個療程。分別采用洼田飲水試驗、才藤榮—吞咽功能障礙評價法評價患者吞咽功能,另通過觀察甘油三酯(TG)、血清白蛋白(ALB)、總膽固醇(TC)、BMI、三頭肌皮褶厚度(TSF)評價患者營養(yǎng)狀況。結(jié)果觀察組洼田飲水試驗評價治愈0例、顯效30例、有效25例、無效3例、總有效率為94.83%,對照組分別為0、20、26、12例和79.31%,兩組總有效率比較,P0.05;觀察組治療前后才藤榮—吞咽功能障礙分級分別為(3.8±0.8)、(5.2±0.7)級,對照組分別為(3.7±0.6)、(4.6±0.7)級,同組治療前后比較,兩組治療后比較,P均0.05。觀察組治療前TG、ALB、TC、BMI、TSF分別為(1.32±0.42)mmol/L、ALB(33.53±2.43)g/L、(4.13±0.24)mmol/L、(21.43±0.4)kg/m2、(14.53±2.19)mm,治療后分別為(1.72±0.32)mmol/L、(39.73±2.66)g/L、(4.63±0.84)mmol/L、(23.40±0.60)kg/m2、(19.64±1.85)mm,治療前后比較,P均0.05;對照組治療前TG、ALB、TC、BMI、TSF分別為(1.29±0.39)mmol/L、(33.69±2.38)g/L、(3.98±0.33)mmol/L、(20.98±0.53)kg/m2、(13.68±2.23)mm,治療后分別為(1.53±0.51)mmol/L、(36.54±2.38)g/L、(4.42±0.25)mmol/L、(22.40±0.50)kg/m2、(16.29±2.25)mm,治療前后ALB、TC、TSF比較,P均0.05;兩組治療后TG、ALB、TC、BMI、TSF比較,P均0.05。結(jié)論酸冰刺激療法聯(lián)合神經(jīng)肌肉電刺激療法治療腦卒中后吞咽障礙療效確切,且進一步改善了患者的營養(yǎng)狀況。
[Abstract]:Objective to observe the effect of acid-ice stimulation combined with neuromuscular electrical stimulation on dysphagia after stroke. Methods 116 patients with dysphagia after stroke were randomly divided into observation group (n = 58) and control group (n = 58). Each course of 20 days, continuous treatment of 2 courses. The patients' swallowing function was evaluated by the test of drinking water in depressions, and the nutritional status of patients was evaluated by observing triglyceride (TGG), serum albumin (ALBN), total cholesterol (TC) and triceps skin fold thickness (TSFs) in order to evaluate the patients' swallowing function, and to evaluate the nutritional status of the patients by observing triglyceride (TG), serum albumin (ALB), total cholesterol (TC), and triceps skin fold thickness (TSF). Results in the observation group, 0 cases were cured, 30 cases were effective, 25 cases were effective and 3 cases were ineffective. The total effective rate of the two groups was 94.83, and that of the control group was 0.20 and 260.12 and 79.31, respectively. The total effective rate of the two groups was higher than that of the control group (P 0.05), the scores of caitongrong and dysphagia in the observation group were 3.8 鹵0.810, 5.2 鹵0.7) and those in the control group were 3.7 鹵0.64.6 鹵0.7, respectively, before and after treatment, and compared before and after treatment.
