【摘要】：目的探究本地區(qū)甲狀腺體積與硒的相關(guān)性及硒營(yíng)養(yǎng)水平是否是甲狀腺腫大及甲狀腺結(jié)節(jié)的危險(xiǎn)因素。方法本文使用分層抽樣的方法調(diào)查了年滿18周歲且在山西太原居住5年以上的居民,共計(jì)1 470人參加了調(diào)查,內(nèi)容包括問(wèn)卷調(diào)查、體格檢查、甲狀腺功能、甲狀腺超聲、尿碘及血硒水平,其中1 339人滿足條件。本文使用了參數(shù)及非參檢驗(yàn)分析數(shù)據(jù)。結(jié)果調(diào)查人群中尿碘的中位數(shù)是:216.4μg/L(四分位數(shù)區(qū)間:139.9μg/L,303.6μg/L),表明碘的營(yíng)養(yǎng)情況處于超足量狀態(tài);血硒的中位數(shù):78.21μg/L(四分位數(shù)區(qū)間:58.46μg/L,128.77μg/L);甲狀腺體積的中位數(shù)7.51 ml(四分位數(shù)區(qū)間5.90 ml,9.39 ml);血硒濃度與年齡呈負(fù)相關(guān)(r=-0.088,P=0.001),與BMI、TSH不相關(guān)(r=-0.007、P=0.801;r=-0.051、P=0.063);在線性回歸分析中甲狀腺體積與血硒無(wú)顯著相關(guān)(β=-0.007,P=0.367);硒在有無(wú)甲狀腺腫大中差異并不顯著(P=0.278,OR=1.173);硒在有無(wú)甲狀腺結(jié)節(jié)中差異也并不顯著(P=0.399,OR=1.073)。結(jié)論硒不影響甲狀腺腫大的發(fā)生,也不是甲狀腺結(jié)節(jié)的影響因素。
[Abstract]:Objective to investigate the correlation between thyroid volume and selenium and whether selenium nutrition is a risk factor for thyroid enlargement and thyroid nodule. Methods stratified sampling was used to investigate the residents who were over 18 years old and lived in Taiyuan, Shanxi province for more than 5 years. A total of 1,470 people participated in the survey, including questionnaire, physical examination, thyroid function, thyroid ultrasound, and so on. Urine iodine and blood selenium levels, of which 1 339 met the condition. Parameters and non-parametric test data are used in this paper. Results the median of urinary iodine in the investigated population was 1: 216.4 渭 g / L (quartile range: 139.9 渭 g / L = 303.6 渭 g / L). The median of blood selenium was 78.21 渭 g / L (quartile: 58.46 渭 g / L = 128.77 渭 g / L); the median of thyroid volume was 7.51 ml (quartile = 5.90 ml ~ 9.39 ml / L); there was a negative correlation between serum selenium concentration and age and no correlation between serum selenium concentration and BMITSH was 0.801r-0.051g / L; in linear regression analysis, there was no correlation between thyroid volume and serum selenium. There was no significant difference in selenium in the presence or absence of thyroidomegaly, and there was no significant difference in selenium in thyroid nodule with or without thyroid nodules, and there was no significant difference in selenium with or without thyroid nodules. Conclusion selenium does not affect the occurrence of goiter and is not a factor of thyroid nodule.
