【摘要】：目的:比較經(jīng)尿道銩激光前列腺剜除術(shù)(TmLEP)與經(jīng)尿道前列腺電切術(shù)(TURP)治療良性前列腺增生癥的安全性及有效性。方法:計算機(jī)檢索CochraneLibrary、PubMed、EMbase、維普、萬方及CNKI等在線數(shù)據(jù)庫,收集比較TmLEP和TURP治療良性前列腺增生癥的隨機(jī)對照試驗(RCT)及病例對照試驗(CCT)。經(jīng)篩選、資料提取和質(zhì)量評價后,采用RevMan 5.1軟件進(jìn)行Meta分析。結(jié)果:共納入5個研究,554例患者。Meta分析結(jié)果顯示:①在安全性方面:與TURP相比,TmLEP術(shù)中出血量較少〔WMD-7.07,CI(-8.99,-5.16)P0.00001〕、血清鈉鹽降低量少〔WMD-3.07,95%CI(-4.36,-1.78)P0.00001〕、輸血人數(shù)比例低〔RR 0.10,CI(0.02,0.42)P=0.002〕、術(shù)后留置導(dǎo)尿時間較短〔WMD-0.92,95%CI(-1.11,-0.73)P0.00001〕、住院天數(shù)較短〔WMD-1.82,95%CI(-3.36,-0.28)P=0.02〕,手術(shù)時間及術(shù)后并發(fā)癥差異無統(tǒng)計學(xué)意義(P0.05);②在有效性性方面:術(shù)后1月TmLEP的最大尿流率(Qmax)評分高于TURP〔WMD-1.37,95%CI(-2.20,-0.53)P=0.01〕,術(shù)后6、12月殘余尿量(PVR)的改善TmLEP優(yōu)于TURP。其它組間指標(biāo)差異均無統(tǒng)計學(xué)意義(P0.05)。結(jié)論:TmLEP與TURP對BPH有相似的療效。TmLEP術(shù)中出血量較少、血清鈉鹽降低量少、輸血人數(shù)少、術(shù)后留置導(dǎo)尿時間較短、住院天數(shù)較短。
