【摘要】：目的:擬觀察在雙側(cè)陰莖海綿體神經(jīng)(CN)損傷的大鼠神經(jīng)性勃起功能障礙(NED)模型中即刻和延時(shí)向陰莖海綿體注射骨髓間充質(zhì)干細(xì)胞(BM-MSCs)對(duì)大鼠陰莖勃起功能的修復(fù)作用。方法:選取28只8周齡、體重200~250 g雄性SD大鼠隨機(jī)分為4組:假手術(shù)組找到雙側(cè)海綿體神經(jīng)后不做處理直接關(guān)腹并縫合皮膚;對(duì)照組、即刻治療組和延時(shí)治療組均通過(guò)鉗夾的方式損傷雙側(cè)陰莖海綿體神經(jīng)建立NED模型隨后關(guān)腹縫合。假手術(shù)組和對(duì)照組向陰莖海綿體內(nèi)注射對(duì)照劑,即刻治療組注射BM-MSCs,延時(shí)治療組術(shù)后兩周注射BM-MSCs。術(shù)后12周采用電刺激CN記錄陰莖海綿體內(nèi)壓(ICP),頸動(dòng)脈穿刺測(cè)定平均動(dòng)脈壓(MAP),ICP/MAP作為勃起功能的評(píng)價(jià)指標(biāo)來(lái)評(píng)估大鼠的勃起功能。在勃起功能檢測(cè)后處死大鼠,取陰莖海綿體中段組織檢測(cè)平滑肌、膠原和神經(jīng)纖維。結(jié)果:在手術(shù)后12周,即刻治療組和延時(shí)治療組ICP和ICP/MAP均較對(duì)照組升高(P0.05)。即刻治療組和延時(shí)治療能提高大鼠海綿體組織內(nèi)平滑肌與膠原的比例(P0.05),同時(shí)兩組陰莖海綿體內(nèi)平滑肌含量高于對(duì)照組(P0.05),陰莖海綿體背神經(jīng)內(nèi)神經(jīng)微絲(NF)蛋白陽(yáng)性神經(jīng)纖維數(shù)目和nNOS的表達(dá)高于對(duì)照組(P0.05)。結(jié)論:陰莖海綿體內(nèi)注射BM-MSCs能對(duì)雙側(cè)CN損傷大鼠的勃起功能恢復(fù)有促進(jìn)作用。BM-MSCs治療可以提高海綿體組織內(nèi)平滑肌與膠原的比例,改善纖維化,并能提高海綿體組織中平滑肌含量、陰莖背神經(jīng)的神經(jīng)絲含量和nNOS的表達(dá)水平,術(shù)后延時(shí)治療同樣能取得一定的治療效果。
[Abstract]:Objective: to observe the injection of bone marrow mesenchymal stem cells (BMSCs) into the cavernous corpus cavernosum of the rat model of neuroerectile dysfunction (NED) in rats with bilateral cavernous nerve injury. Methods: 28 rats (8 weeks old) were selected to repair the penile erectile function. Male SD rats weighing 200 ~ 250g were randomly divided into 4 groups: sham operation group found bilateral cavernous nerves and closed abdomen and sutured skin without treatment; In the control group, immediate treatment group and delayed treatment group, the bilateral cavernous nerve was injured by clamp to establish NED model and then closed abdominal suture. The sham operation group and control group were injected into the corpus cavernosum. BM-MSCs was injected into the immediate treatment group, and BM-MSCs was injected into the delayed treatment group two weeks after operation. ICP was recorded by electrical stimulation of CN 12 weeks after operation. The average arterial pressure (MAPP / map) was measured by carotid puncture to evaluate the erectile function of rats. The rats were killed after the examination of erectile function. Smooth muscle collagen and nerve fibers were measured in the middle part of the corpus cavernosum. Results: 12 weeks after operation. ICP and ICP/MAP in immediate treatment group and delayed treatment group were higher than those in control group (P 0.05). The ratio of smooth muscle to collagen in cavernous tissue was increased in immediate treatment group and delayed treatment group (2). P0.05). At the same time, the content of smooth muscle in corpus cavernosum of both groups was higher than that of control group (P0.05). The number of positive nerve fibers and the expression of nNOS in the dorsal cavernous nerve of the penis were higher than those in the control group (P0.05). ... conclusion:. Intracavernous injection of BM-MSCs can promote the recovery of erectile function in bilateral CN injured rats. BM-MSCs can improve the ratio of smooth muscle to collagen in cavernous tissue. It can improve fibrosis and increase the content of smooth muscle in cavernous body, the content of neurofilament and the expression of nNOS in dorsal penile nerve.
