【摘要】：目的觀察評(píng)價(jià)羅哌卡因應(yīng)用于下頜阻生智齒拔除的麻醉效果。方法將2016年1—6月首都醫(yī)科大學(xué)附屬北京友誼醫(yī)院口腔科就診的150例需要拔除下頜阻生智齒且無(wú)其他系統(tǒng)疾病的患者隨機(jī)分成2組:試驗(yàn)組(71例)采用0.75%羅哌卡因進(jìn)行局部麻醉,對(duì)照組(79例)則采用2%利多卡因進(jìn)行局部麻醉。記錄麻醉起效時(shí)間、麻醉持續(xù)時(shí)間、拔牙術(shù)后不同時(shí)間點(diǎn)的疼痛程度(VAS分級(jí))、術(shù)后患者是否主動(dòng)要求服用止痛藥、麻醉前后不同時(shí)間點(diǎn)患者血壓及脈搏,并對(duì)麻醉并發(fā)癥進(jìn)行監(jiān)測(cè)記錄,從而比較兩種藥物的麻醉效果。結(jié)果試驗(yàn)組麻醉起效時(shí)間明顯短于對(duì)照組(P0.05),而麻醉持續(xù)時(shí)間明顯長(zhǎng)于對(duì)照組(P0.05);試驗(yàn)組患者術(shù)后不同時(shí)間點(diǎn)VAS分級(jí)均明顯低于對(duì)照組(P0.05),且患者術(shù)后主動(dòng)要求服用止痛藥的比率明顯低于對(duì)照組(P0.05);對(duì)照組患者血壓及脈搏在麻藥注射結(jié)束后各個(gè)時(shí)間點(diǎn)較注射前均無(wú)明顯變化(P0.05);試驗(yàn)組血壓在麻藥注射后各個(gè)時(shí)間點(diǎn)較注射前無(wú)明顯變化(P0.05),脈搏在注射后30 min有小幅度下降(P0.05);兩組患者注射麻藥后均未產(chǎn)生麻醉并發(fā)癥。結(jié)論羅哌卡因因其麻醉起效時(shí)間短,麻醉持續(xù)時(shí)間長(zhǎng),術(shù)后陣痛效果好,心血管系統(tǒng)風(fēng)險(xiǎn)小等優(yōu)點(diǎn)可廣泛應(yīng)用于時(shí)間較長(zhǎng)的復(fù)雜口腔專(zhuān)業(yè)操作。
[Abstract]:Objective To evaluate the effect of rapivacaine on the extraction of mandibular impacted wisdom teeth. Methods 150 patients in the department of stomatology of Beijing Friendship Hospital from January to June 2016 were randomly divided into two groups: the test group (71 cases) was given the local anesthesia with 0.75% Robupivacaine. In the control group (79 cases),2% lidocaine was used for local anesthesia. To record the time of onset of anesthesia, the duration of anesthesia, the degree of pain at different time points after tooth extraction (VAS classification), whether the patient actively required to take the pain medication, the blood pressure and the pulse of the patient at different time points before and after anesthesia, and the monitoring and recording of the anesthesia complications, So as to compare the anesthetic effect of the two drugs. Results The duration of anesthesia in the test group was significantly shorter than that of the control group (P0.05), and the duration of the anesthesia was significantly longer than that of the control group (P0.05). And the blood pressure and pulse of the control group were significantly lower than that of the control group (P0.05). The blood pressure of the test group did not change significantly before the injection of the anesthetic (P0.05), and the pulse amplitude decreased at 30 min after injection (P0.05); no anesthetic complications occurred after the injection of the anesthetic in the two groups. Conclusion Robupivacaine has the advantages of short duration of anesthesia, long duration of anesthesia, good effect of postoperative pain and small risk of cardiovascular system, and can be widely used in complex oral operation with long time.
