【摘要】：目的評(píng)價(jià)0~4歲兒童接種乙肝疫苗的免疫效果,探討乙肝疫苗加強(qiáng)免疫可行性。方法采用多級(jí)、隨機(jī)整群抽樣方法,抽取蘇州市部分市、區(qū)0~4歲已接種乙肝疫苗的兒童為研究對(duì)象,檢測(cè)血清中乙肝病毒5項(xiàng)血清學(xué)標(biāo)志物。結(jié)果本次調(diào)查蘇州市的877名兒童,乙肝疫苗全程接種率93.16%,0~4歲兒童HBsAg陽(yáng)性率為0.11%,HBsAb陽(yáng)性率為74.57%,HBeAg陽(yáng)性率為0,HBeAb陽(yáng)性率為2.17%,HBcAb為1.25%。不同年齡組間HBsAb陽(yáng)性率差異有統(tǒng)計(jì)學(xué)意義(χ2=134.33,P0.01),HBsAb陽(yáng)性率隨著年齡的增長(zhǎng)而呈逐年下降趨勢(shì)(χ2趨勢(shì)性檢驗(yàn)=108.24,P0.01)。HBsAb陽(yáng)性率與年齡、免疫次數(shù)和全程是否及時(shí)等因素有關(guān)。結(jié)論蘇州市0~4歲兒童乙肝疫苗全程接種率較高,HBsAb陽(yáng)性率隨著年齡的增長(zhǎng)而呈逐年下降趨勢(shì)。建議本地區(qū)可推行4歲及以上兒童乙肝疫苗加強(qiáng)免疫策略。
[Abstract]:Objective to evaluate the immune effect of hepatitis B vaccine in children aged 0 ~ 4 years, and to explore the feasibility of hepatitis B vaccine immunization. Methods multistage random cluster sampling was used to detect 5 serological markers of hepatitis B virus in serum of children who had been vaccinated with hepatitis B vaccine in 4 years old in some cities and districts of Suzhou. Results among 877 children in Suzhou, the rate of hepatitis B vaccine vaccination was 93.160.The positive rate of HBsAg in children aged 4 years was 0.11 and the positive rate of HBeAg was 74.577.The positive rate of HBeAg was 2.17 and the positive rate of HBcAb was 1.25. There was a significant difference in HBsAb positive rate among different age groups (蠂 ~ 2 / 134.33 / P ~ (0.01), HBsAb positive rate decreased with age (蠂 ~ (2) trend test = 108.24p _ (0.01) P _ (0.01). HBsAb positive rate was related to age, times of immunization and whether the whole process was timely or not). Conclusion the total coverage rate of hepatitis B vaccine in children aged 0 to 4 years old in Suzhou is higher, and the positive rate of HBsAb is decreasing year by year with the increase of age. It is suggested that children aged 4 years and above should be immunized against hepatitis B vaccine.
