【摘要】：目的：采用免疫組化法檢測(cè)L-選擇素（L-Selectin）在口腔鱗狀細(xì)胞癌（OralSquamous Cell Carcinoma，OSCC）組織和涎腺惡性腫瘤（Salivary gland malignanttumor）組織中的表達(dá)情況，并初步探討L-選擇素與口腔鱗狀細(xì)胞癌和涎腺惡性腫瘤的發(fā)生、發(fā)展和侵襲轉(zhuǎn)移的相關(guān)性。 方法：收集佳木斯大學(xué)第二附屬醫(yī)院·口腔醫(yī)院病理科保存的手術(shù)切除的口腔鱗狀細(xì)胞癌石蠟標(biāo)本48例，涎腺惡性腫瘤石蠟標(biāo)本48例，，另選擇因口腔頜面部良性病變而切除的病變組織周圍的正�？谇火つそM織和正常涎腺組織各20例，分別作為對(duì)照組。采用鏈霉菌抗生物素蛋白-過氧化酶(SP)免疫組織化學(xué)方法檢測(cè)L-選擇素的表達(dá)情況，采用Concise Statistics，CS10.34統(tǒng)計(jì)軟件進(jìn)行統(tǒng)計(jì)學(xué)處理，探討L-選擇素的表達(dá)情況與口腔鱗狀細(xì)胞癌和涎腺惡性腫瘤的各臨床病理特征之間的關(guān)系。 結(jié)果： 1.L-選擇素在口腔鱗狀細(xì)胞癌中的陽(yáng)性表達(dá)率約為41.7%（20/48），明顯高于其對(duì)照組的10%（2/20）（P0.05）。 2.L-選擇素在涎腺惡性腫瘤中的陽(yáng)性表達(dá)率約為68.8%（33/48），明顯高于其對(duì)照組的10%（2/20）（P0.05）。 3.在口腔鱗狀細(xì)胞癌組織中，L-選擇素的表達(dá)與患者口腔鱗狀細(xì)胞癌組織的分化程度、TNM分期和頸淋巴結(jié)轉(zhuǎn)移有關(guān)；與患者的性別、年齡無(wú)關(guān)。 4.在涎腺惡性腫瘤組織中，L-選擇素的表達(dá)與患者涎腺惡性腫瘤的頸淋巴結(jié)轉(zhuǎn)移有關(guān)；與患者涎腺惡性腫瘤的組織分類，性別、年齡無(wú)關(guān)。 5.L-選擇素在口腔鱗狀細(xì)胞癌組織與涎腺惡性腫瘤組織中的表達(dá)顯示，涎腺惡性腫瘤組織中的L-選擇素陽(yáng)性率明顯高于其在鱗狀細(xì)胞癌組織中的陽(yáng)性率（P＜0.05）。 結(jié)論：L-選擇素在口腔鱗狀細(xì)胞癌組織和涎腺惡性腫瘤組織中均存在著高表達(dá)；L-選擇素在口腔鱗狀細(xì)胞癌和涎腺惡性腫瘤的發(fā)生、發(fā)展和侵襲轉(zhuǎn)移中起著重要作用；L-選擇素有望作為口腔頜面部惡性腫瘤早期診斷的標(biāo)志。
[Abstract]:Objective: to detect the expression of L-selectin (L-Selectin) in oral squamous cell carcinoma (OralSquamous Cell Carcinoma,OSCC) and salivary gland malignant tumor (Salivary gland malignanttumor) by immunohistochemical method. The relationship between L- selectin and the occurrence, development, invasion and metastasis of oral squamous cell carcinoma and salivary gland malignant tumor was studied. Methods: the paraffin specimens of 48 cases of oral squamous cell carcinoma and 48 cases of malignant tumors of salivary gland were collected from Department of Stomatology, second affiliated Hospital of Jiamusi University. In addition, 20 cases of normal oral mucosa and 20 cases of normal salivary gland were selected as control group. The expression of L-selectin was detected by streptomyces biotin-peroxidase (SP) immunohistochemical method. The expression of L-selectin was analyzed by Concise Statistics,CS10.34 software. To investigate the relationship between the expression of L-selectin and the clinicopathological features of oral squamous cell carcinoma and salivary gland malignant tumor. Results: 1. The positive expression rate of L- selectin in oral squamous cell carcinoma was 41.7% (20 / 48), which was significantly higher than that in control group (10%, 2 / 20) (P0.05). 2. The positive expression rate of L- selectin in salivary gland malignant tumor was 68.8% (33 / 48), which was significantly higher than that in control group (10%, 2 / 20) (P0.05). 3. In oral squamous cell carcinoma (OSCC), the expression of L- selectin was related to the degree of differentiation, TNM stage and cervical lymph node metastasis, but not to sex and age. 4. The expression of L- selectin was related to cervical lymph node metastasis in salivary gland malignant tumors, but not to the classification, sex and age of salivary gland malignant tumors. 5. The expression of L- selectin in oral squamous cell carcinoma and salivary gland malignant tumor showed that the positive rate of L- selectin in salivary gland malignant tumor was significantly higher than that in squamous cell carcinoma (P < 0.05). Conclusion: L- selectin is highly expressed in oral squamous cell carcinoma and salivary gland malignant tumor, and it plays an important role in the occurrence, development, invasion and metastasis of oral squamous cell carcinoma and salivary gland malignant tumor. L-selectin is expected to be a marker for early diagnosis of oral and maxillofacial malignant tumors.
