【摘要】：目的：研究顳下頜關(guān)節(jié)可復(fù)性關(guān)節(jié)盤前移位(Anterior Disc Displacement with Reduction, ADDR)患者經(jīng)過保守治療,關(guān)節(jié)彈響癥狀消失前后關(guān)節(jié)盤的位置變化,探討關(guān)節(jié)彈響與關(guān)節(jié)盤位置的關(guān)系。 方法：選取27例要求治療顳下頜關(guān)節(jié)彈響的患者,其中49側(cè)關(guān)節(jié)診斷為ADDR,采用口頜系統(tǒng)功能鍛煉治療,分別行治療前和經(jīng)治療彈響消失后雙側(cè)顳下頜關(guān)節(jié)MRI掃描,并錄像記錄患者開閉口時下頜運動狀態(tài)。選取TMJ斜矢狀位MRI采用鐘點法評價關(guān)節(jié)盤位置,對所記錄數(shù)據(jù)采用SPSS13.0軟件進(jìn)行t檢驗統(tǒng)計學(xué)分析。 結(jié)果：治療前關(guān)節(jié)盤位置為28.15°±10.92°,其中4側(cè)關(guān)節(jié)有彈響癥狀但盤位正常,45側(cè)關(guān)節(jié)有彈響癥狀且關(guān)節(jié)盤前移位,關(guān)節(jié)盤前移率為91.84%。經(jīng)治療彈響消失后關(guān)節(jié)盤位置為28.64°±6.65°,治療前評價為盤前移位的45側(cè)關(guān)節(jié)在治療后仍為盤前移位。治療前與治療后對比,關(guān)節(jié)盤位置無顯著性差異(P0.05)。 結(jié)論：顳下頜關(guān)節(jié)彈響與關(guān)節(jié)盤的位置是否前移無關(guān)。
[Abstract]:Objective: to study the changes of the position of the articular disc before and after conservative treatment in the patients with Anterior disc displacement with reduction (ADDRs), and to explore the relationship between the joint elastic response and the position of the articular disc. Methods: Twenty-seven patients with temporomandibular joint (TMJ) bounce were selected, 49 of them were diagnosed as ADDR.The patients were treated with oral and maxillary system functional exercise. MRI scans of bilateral temporomandibular joint were performed before and after the treatment. The mandibular movement was recorded by video recording. TMJ oblique sagittal MRI was used to evaluate the position of articular disc by hour method, and the recorded data were analyzed by t test with SPSS 13.0 software. Results: the position of articular disc before treatment was 28.15 擄鹵10.92 擄, of which 4 joints had bounce symptoms but 45 joints in normal disc position had elastic symptoms and anterior disc displacement. The anterior displacement rate of disc was 91.84%. After treatment, the position of articular disc was 28.64 擄鹵6.65 擄. 45 joints with anterior disc displacement were still anterior disc displacement after treatment. There was no significant difference in the position of articular disc before and after treatment (P0.05). Conclusion: the response of temporomandibular joint is not related to the position of disc.
