【摘要】：目的·探索孤獨癥譜系障礙(ASD)兒童胃腸問題檢出率及分布情況。方法·納入在上海交通大學醫(yī)學院附屬上海兒童醫(yī)學中心及附屬新華醫(yī)院發(fā)育行為兒科就診的3~8歲ASD患兒336名。所有患兒均符合《精神疾病診斷與統(tǒng)計手冊》第5版診斷標準。由父母或照養(yǎng)人填寫患兒一般資料,包括出生年月、性別及近3個月胃腸問題現(xiàn)況等。探索胃腸問題在此人群中的檢出率及分布情況。結(jié)果·336例ASD患兒中,胃腸問題陽性85例,總體檢出率25.3%。其中便秘59例(17.6%),嘔吐12例(3.6%),腹瀉11例(3.3%),腹痛7例(2.1%),腹脹6例(1.8%)。在3~8歲合并胃腸問題的ASD兒童中,不同年齡組之間胃腸問題檢出率的差異無統(tǒng)計學意義(χ~2=1.511,P=0.680),胃腸問題嚴重度的差異也無統(tǒng)計學意義(F=0.773,P=0.513)。結(jié)論·胃腸問題在ASD兒童中較常見,且在3~8歲年齡段持續(xù)存在,其嚴重度不隨年齡增長而降低。提示ASD兒童胃腸問題,尤其是便秘,需作為慢病管理的一部分,其評估和治療值得臨床關(guān)注。
[Abstract]:Objective to investigate the prevalence and distribution of gastrointestinal problems in children with autism spectrum disorder (ASD). Methods A total of 336 children with ASD aged 3 years old from Shanghai Children's Medical Center and affiliated Xinhua Hospital of Shanghai Jiaotong University were enrolled in this study. All the children were in accordance with the fifth edition of the Diagnostic and Statistical Manual for Mental Disorders. General information of the child, including year of birth, sex and status of gastrointestinal problems in the last 3 months, were filled in by parents or foster persons. To explore the detection rate and distribution of gastrointestinal problems in this population. Results of 336 cases of ASD, 85 cases were positive for gastrointestinal problems, the total detection rate was 25.3%. Among them, 59 cases had constipation, 12 cases had vomiting, 11 cases had diarrhea, 7 cases had abdominal pain, 7 cases had abdominal pain, and 6 cases had abdominal distention. There was no significant difference in the detection rate of gastrointestinal problems among different age groups in ASD children aged 3 to 8 years (蠂 ~ (2) 2 ~ (1.511) P ~ (0.680)), and there was no significant difference in the severity of gastrointestinal problems (P < 0.05 ~ (13)). Conclusion Gastrointestinal problems are more common in children with ASD and persist in the age group of 3 to 8 years. The severity of gastrointestinal problems does not decrease with the increase of age. It is suggested that gastrointestinal problems, especially constipation, in children with ASD should be considered as part of management of chronic diseases, and its evaluation and treatment are worthy of clinical attention.
