【摘要】：背景:根據既往文獻報道,酒精使用障礙是精神分裂癥患者最常見的共患障礙之一,精神分裂癥患者同時罹患酒精使用障礙的比率是普通人群的3倍。精神分裂癥共患酒精使用障礙往往導致治療效果和治療依從性更差,病情更易復發(fā)和再住院,增加暴力沖動和自殺行為的風險,然而我國精神分裂癥患者的酒精使用狀況卻缺乏系統(tǒng)的調查。目的:了解精神分裂癥患者住院前的飲酒狀況及其酒精使用障礙的共病率。通過精神分裂癥共患酒精使用障礙者的人口學特征分析、推斷精神分裂癥共患酒精使用障礙的相關因素和高危人群,為制定預控精神分裂癥患者的酒精使用障礙提供科學依據。方法:采用分層整群抽樣的方法,應用自編飲酒調查表、酒精使用障礙篩查量表(AUDIT)對542名住院精神分裂癥患者院前的飲酒狀況進行調查,根據美國精神障礙診斷與統(tǒng)計手冊第5版(DSM-5)酒精使用障礙標準對篩查陽性者(AUDIT≥7)進行診斷。所有調查數據均在SPSS13.0統(tǒng)計軟件包上進行統(tǒng)計分析。結果:精神分裂癥患者男、女及總樣本終生飲酒率分別是65.1%、42.2%、5 5.9%,年飲酒率分別是40.4%、27.2%、35.1%;年飲酒者中男、女及總樣本AUDIT陽性率分別是40.3%、12.5%、31.7%。飲酒者中平均每次飲酒為4 2.3 6g(中位數)純酒精,男:女為4.0,啤酒和低度白酒是本地區(qū)精神分裂癥患者最常飲用的酒類。男、女及總樣本酒精使用障礙時點共病率分別是9.1%、2.0%、6.2%,男性高于女性(x2=10.86,P=0.001)。結論:飲酒是本地區(qū)精神分裂癥患者的一種普遍的社會行為,啤酒和低度白酒是本地區(qū)精神分裂癥患者消費的主流,精神分裂癥患者的飲酒率低于普通人群,且合并酒精使用障礙的診斷率低于西方發(fā)達國家。
