【摘要】：目的觀察神經(jīng)節(jié)苷酯鈉治療急性重癥顱腦損傷的臨床療效。方法將90例急性重癥顱腦損傷患者隨機(jī)分為對照組45例和試驗(yàn)組45例。對照組予以靜脈滴注5 mL·kg~(-1)20%甘露醇,30 min完畢+0.4 mg·kg~(-1)呋塞米+納洛酮8 mg+醒腦靜注射液20 mL+注射用血凝酶1 kU,連續(xù)治療8周。試驗(yàn)組在對照組的基礎(chǔ)上,加用靜脈滴注神經(jīng)節(jié)苷脂鈉注射液100 mg,連續(xù)治療2周后,改為40 mg·d~(-1)維持治療6周。比較2組患者的炎性因子、凝血功能及預(yù)后情況。結(jié)果治療后,試驗(yàn)組的降鈣素基因相關(guān)肽明顯高于對照組,腫瘤壞死因子、內(nèi)皮素明顯低于對照組(P0.05);試驗(yàn)組的凝血酶時(shí)間明顯低于對照組,纖維蛋白原明顯高于對照組(P0.05),但2組患者的血小板、凝血酶原時(shí)間、活化部分凝血酶時(shí)間等指標(biāo)比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。試驗(yàn)組的預(yù)后良好率為46.67%,明顯高于對照組22.22%(P0.05)。結(jié)論神經(jīng)節(jié)苷酯鈉治療急性重癥顱腦損傷有助于緩解炎性癥狀,改善凝血功能,提高臨床療效。
[Abstract]:Objective to observe the clinical effect of ganglioside sodium in the treatment of acute severe craniocerebral injury. Methods 90 patients with acute severe craniocerebral injury were randomly divided into control group (n = 45) and experimental group (n = 45). The control group was given 5 mL kg~ (-1) 20% mannitol intravenously. After 30 min, 0.4 mg kg~ (-1) furosemide 8 mg Xingnaojing injection (20 mL) was injected with hemocoagulase 1 kU, for 8 weeks. On the basis of the control group, the experimental group was treated by intravenous injection of ganglioside sodium injection for 2 weeks, then changed to 40 mg D1 for 6 weeks. The inflammatory factors, coagulation function and prognosis of the two groups were compared. Results after treatment, calcitonin gene-related peptide in the experimental group was significantly higher than that in the control group, tumor necrosis factor and endothelin were significantly lower than those in the control group (P0.05). The thrombin time and fibrinogen in the experimental group were significantly lower than those in the control group (P0.05), but the platelet and prothrombin time of the two groups were significantly higher than that of the control group (P0.05). There was no significant difference in activated partial thrombin time (P0.05). The good prognosis rate of the trial group was 46.67, significantly higher than that of the control group 22.22% (P0.05). Conclusion ganglioside sodium can relieve inflammatory symptoms, improve coagulation function and improve clinical efficacy in patients with acute severe craniocerebral injury.
