【摘要】：目的觀察心臟手術(shù)后無(wú)菌性全身炎癥反應(yīng)綜合征(SIRS)繼發(fā)多器官功能障礙綜合征(MODS)患者血清CRP、N端腦鈉肽(NT-pro BNP)水平變化,探討其與患者病情及預(yù)后的關(guān)系。方法選擇行心臟手術(shù)后發(fā)生無(wú)菌性SIRS繼發(fā)MODS患者35例(觀察組),同期行心臟手術(shù)后未發(fā)生MODS患者35例(對(duì)照組)。采用AU5400全自動(dòng)生化分析儀檢測(cè)兩組血清CRP、NT-pro BNP,分析觀察組MODS評(píng)分、住院期間生存情況與血清CRP、NT-pro BNP的關(guān)系。結(jié)果觀察組血清CRP水平明顯高于對(duì)照組(P0.05),兩組血清NT-pro BNP水平比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組血清CRP與MODS評(píng)分呈正相關(guān)(r=0.918,P0.05),血清NT-pro BNP與MODS評(píng)分無(wú)明顯相關(guān)性(r=0.297,P0.05)。觀察組術(shù)后住院時(shí)間(21±5)d,對(duì)照組為(8±2)d,兩組比較P0.05。觀察組住院期間存活29例、死亡6例,存活者血清CRP水平明顯低于死亡者(P0.05),不同預(yù)后者血清NT-pro BNP水平比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論心臟手術(shù)后無(wú)菌性SIRS繼發(fā)MODS患者血清CRP水平升高,血清NT-pro BNP水平無(wú)明顯變化;血清CRP水平有助于評(píng)估患者病情及判斷預(yù)后。
[Abstract]:Objective to observe the changes of serum CRP,N terminal brain natriuretic peptide (NT-pro BNP) levels in patients with aseptic systemic inflammatory response syndrome (SIRS) secondary to multiple organ dysfunction syndrome (MODS) after cardiac surgery. Methods 35 cases of MODS secondary to aseptic SIRS were selected after cardiac surgery (observation group) and 35 cases of patients without MODS after cardiac surgery (control group). AU5400 automatic biochemistry analyzer was used to detect the MODS score of the two groups of serum CRP,NT-pro BNP, analysis observation group, and the relationship between the survival condition and serum CRP,NT-pro BNP during hospitalization. Results the serum CRP level in the observation group was significantly higher than that in the control group (P0.05). There was no significant difference in the serum NT-pro BNP level between the two groups (P0.05). There was a positive correlation between serum CRP and MODS score in the observation group (r = 0.918 P 0.05), but no significant correlation between the serum NT-pro BNP score and the MODS score (r = 0.297 P 0.05). The hospital stay was (21 鹵5) d in the observation group and (8 鹵2) d in the control group (P < 0.05). In the observation group, 29 cases survived and 6 cases died. The serum CRP level of the survivors was significantly lower than that of the dead (P0.05). There was no significant difference in the serum NT-pro BNP levels between the patients with different prognosis (P0.05). Conclusion after cardiac surgery, the level of serum CRP in patients with MODS secondary to aseptic SIRS is increased, but the level of serum NT-pro BNP does not change significantly, and the level of serum CRP is helpful to evaluate the condition of the patients and to judge the prognosis.
