【摘要】：目的探討圍生期因素、喂養(yǎng)方式、服用益生菌、發(fā)病前合并疾病及發(fā)病前接受機(jī)械通氣等對新生兒壞死性小腸結(jié)腸炎(NEC)發(fā)病的影響,旨在提供NEC發(fā)生的高危因素。方法選取2011年1月至2016年1月于中國醫(yī)科大學(xué)附屬盛京醫(yī)院第一新生兒病房住院治療的76例NEC患兒為病例組,并隨機(jī)選取同期住院的80例非NEC患兒為對照組�；仡櫺苑治龌純旱呐R床資料,包括新生兒一般情況、圍生期情況、喂養(yǎng)方式、口服益生菌情況、發(fā)病前合并其他疾病情況及接受機(jī)械通氣等19個(gè)項(xiàng)目,應(yīng)用SPSS軟件進(jìn)行單因素及多因素分析。結(jié)果單因素分析結(jié)果顯示,病例組合并窒息、敗血癥、低血糖以及接受機(jī)械通氣的比例顯著高于對照組,口服益生菌的比例顯著低于對照組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。Logistic多元回歸分析結(jié)果顯示,敗血癥(OR=6.531)、低血糖(OR=2.785)、機(jī)械通氣(OR=3.448)為NEC發(fā)病的危險(xiǎn)因素,口服益生菌為NEC的保護(hù)因素(OR=0.417)。結(jié)論敗血癥、低血糖及機(jī)械通氣為NEC發(fā)病的危險(xiǎn)因素,口服益生菌為NEC的保護(hù)因素。臨床上應(yīng)合理分析這些影響因素,早期預(yù)防及干預(yù)NEC,以改善患兒預(yù)后。
[Abstract]:Objective to investigate the effects of perinatal factors, feeding methods, probiotics, premorbid diseases and mechanical ventilation on the incidence of neonatal necrotizing enterocolitis (NEC) in order to provide high risk factors for the occurrence of NEC. Methods from January 2011 to January 2016, 76 cases of NEC in the first neonatal ward of Shengjing Hospital affiliated to China Medical University were selected as the case group, and 80 cases of non-NEC in the same period were randomly selected as the control group. The clinical data of the children were analyzed retrospectively, including 19 items, such as general condition of newborn, perinatal period, feeding method, oral probiotics, other diseases before onset and mechanical ventilation. Single factor and multi-factor analysis were carried out with SPSS software. Results the results of univariate analysis showed that the proportion of cases with asphyxia, sepsis, hypoglycemia and mechanical ventilation was significantly higher than that of the control group, and the proportion of oral probiotics was significantly lower than that of the control group. Logistic multiple regression analysis showed that septicemia (OR=6.531), hypoglycemia (OR=2.785), mechanical ventilation (OR=3.448) were the risk factors of NEC, and oral probiotics was the protective factor of NEC (OR=0.417). Conclusion septicemia, hypoglycemia and mechanical ventilation are risk factors of NEC, oral probiotics are the protective factors of NEC. In order to improve the prognosis of children, we should analyze these influencing factors reasonably and prevent and intervene NECs early.
