【摘要】：目的:分析小兒流感病毒A型肺炎的臨床特征,觀察流感病毒A型肺炎感染與中醫(yī)辨證分型之間的關系,以期為小兒流感病毒A型肺炎的臨床診療分型提供辨證依據(jù)。方法:搜集150例2015年12月1日~2016年12月1日山東中醫(yī)藥大學附屬醫(yī)院兒科流感病毒A型肺炎住院患兒的病例資料,用SPSS20軟件統(tǒng)計并分析小兒流感病毒A型肺炎臨床特點及其與中醫(yī)證型、體質分布之間的關系。結果:小兒流感病毒A型肺炎有明顯的年齡相關性,幼兒及學齡前兒童較易發(fā)病;發(fā)病一般以秋、冬季節(jié)為主;且發(fā)熱較高,病程較長;一般患流感病毒A型肺炎患兒均伴有高熱;流感病毒A容易與其他病原體混合致病;血象以白細胞數(shù)值正常為主;各中醫(yī)證型在發(fā)病年齡、病程、季節(jié)上有顯著差異(P0.01),在體質方面存在差異(P0.05)。結論:小兒流感病毒A型肺炎具有其獨特的發(fā)病特點,其發(fā)病與年齡、季節(jié)、體質密切相關;小兒流感病毒A型肺炎亦有其獨特的中醫(yī)證候學特征,以濕熱閉肺證、風熱閉肺證、痰熱閉肺證多見,流感病毒A型肺炎患兒體質以肺脾質II型多見,掌握其發(fā)病的臨床特點與中醫(yī)辨證規(guī)律有助于提高臨床診療效果,具有臨床應用價值。
[Abstract]:Objective: to analyze the clinical characteristics of infantile influenza virus A pneumonia and to observe the relationship between influenza virus A pneumonia infection and TCM syndrome differentiation in order to provide evidence for clinical diagnosis and treatment of influenza A pneumonia in children. Methods: 150 hospitalized children with influenza virus A pneumonia were collected from the affiliated Hospital of Shandong University of traditional Chinese Medicine from December 1, 2015 to December 1, 2016. The clinical characteristics of infantile influenza virus type A pneumonia and its relationship with TCM syndromes and physical distribution were analyzed by SPSS20 software. Results: there was a significant age correlation between children with influenza virus A pneumonia, and children and preschool children were more susceptible to the disease, the disease was mainly in autumn and winter, and the fever was higher and the course of disease was longer. In general, children with influenza virus type A pneumonia are accompanied with high fever; influenza virus A is easily mixed with other pathogens to cause disease; the hematology is dominated by normal leukocyte count; each TCM syndrome type is at the onset age and course of disease. There were significant differences in the season (P 0.01) and in physique (P 0.05). Conclusion: infantile influenza virus A pneumonia has its unique characteristics, and its incidence is closely related to age, season and physique, and infantile influenza virus A pneumonia also has its unique TCM syndromes, which is damp-heat blocking lung syndrome. Wind-heat blocking lung syndrome, phlegm heat blocking lung syndrome are more common, influenza virus type A pneumonia children physique in type II of lung and spleen, mastering the clinical characteristics of the disease and the law of TCM syndrome differentiation are helpful to improve the clinical diagnosis and treatment effect and have clinical application value.
